The Justice & Diversity Center of The Bar Association of San Francisco (JDC) has an exciting opportunity for an Intensive Case Manager position at our Homeless Advocacy Project (HAP).
HAP works with tenants who are facing unlawful detainers (evictions,) prioritizing clients who have mental and/or physical disabilities, and tenants living in the 94102 (Tenderloin) zip code. HAP's Eviction Defense Team provides full scope representation (start to finish) for tenants in unlawful detainers. The HAP holistic approach supports clients in maintaining their housing by addressing multiple facets of their lives to tackle underlying problems that have placed them at risk.
JDC is a non-profit organization that provides free legal services to low-income people in San Francisco, and to other nonprofits that serve underrepresented communities. We do this work in part through directly representing clients and in part through recruiting private practice attorneys and legal professionals to assist clients pro bono. JDC's staff and volunteer attorneys serve over 8,500 low-income people a year, many of whom face loss of their housing, income or disability benefits, or divorce and child custody issues. JDC's holistic advocacy approach addresses clients' legal and social service needs to help them achieve lasting change in their lives.
About the Job:
Work alongside HAP Staff Attorneys to provide outreach, engagement and coordination of services to eviction clients both within HAP and with outside organizations.
A. Conduct home visits and coordinate client communication and engagement, including but not limited to: Assessing living conditions and connecting clients with appropriate resources to maintain their unit
B. Locating clients who have not responded to correspondence from the HAP Attorney regarding status of case, notifying clients of upcoming hearings where the client needs to be present, arranging for and accompanying clients to appointments when needed to insure their presence
C. Coordinating referrals for benefits such as General Assistance, CalFresh, and SSI, Meals on Wheels, as well as rental assistance and payee services
D. Assisting clients to get to related appointments to address underlying issues that effect their housing case, such as mental health appointments, rental assistance, or benefits related issues
E. Providing emotional and practical support to clients to help them engage effectively with their case, gain stability in their lives and control behavior issues that contributed to the eviction
Attend weekly meeting with HAP Eviction Attorneys
Meet regularly with JDC Director of Social Services to discuss appropriate and effective approaches to meeting clients' social service needs
Maintain timely notes and records for tenant case file, and be in consistent communication with HAP Staff Attorneys
EDUCATION & EXPERIENCE:
BA or MSW in Social Work or related field
At least 2 years of experience providing case management to clients with significant mental health and/or substance abuse issues.
Knowledge of trauma-informed approaches to care and familiarity with resources in San Francisco
SKILLS & REQUIREMENTS:
Strong oral, written, and client service skills
Ability to work independently, and to function well in a hectic and sometimes challenging environment.

About the Role

Qualifications for our Therapists:
Experience as an independently Licensed Clinical Social Worker, Licensed Professional Counselor, and or Licensed Marriage and Family Therapist
Must have passed the ASWB, MFT, NCE, and/or NCMHCE exam to obtain clinical licensure.
Reside in the U.S. or a U.S. territory.

The RoleThis is a general open application to clinically licensed therapists in California. Mental Health Counseling is part of a broad range of TimelyCare's digital health services that are extended to members of higher education, primarily college students. As a 1099 Licensed Clinical Therapist with TimelyCare, you will offer therapy for the members we serve, partnering with them to identify their needs, collaborate and develop treatment plans, and guide them, when necessary, to other appropriate TimelyCare services (Medical, Psychiatry, etc.).
We have a mental health team that consists of Licensed Clinical Social Workers, Professional Counselors, Marriage and Family Therapists, and Clinical Psychologists. Requirements for Contracted Services
Required: Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
Associate therapists are not applicable for this position at this time

Strong clinical documentation, assessment, and active listening skills

What Our Contracted Therapists Do
Provide therapeutic counseling services and treatment planning to a primarily young adult population
Document concurrently and submit HIPAA compliant documentation using TimelyCare's electronic health record
Develop and submit treatment plans and progress notes following each session
Communicate cross functionally with other teams/departments in order to support care continuation as needed
Adhere to established standards for documentation quality and code of ethics
Expectations for Contracted Services
Work with the student/client to establish and develop treatment goals and document accordingly
Utilize active listening skills and evidence based modalities (SFBT, CBT, DBT, MI, ACT) in order to build rapport, establish boundaries, and progress student/client care
Submit appropriate documentation in the EHR (the TimelyCare platform) in a punctual manner
Prepared to adapt to the changing needs of the population we serve
Able to remain calm and maintain composure during a potential high stress scenario
Respond with empathy, professionalism, and meet clients where they are at
Model respectful conduct with all students, peers, and support staff

The Providence Center Clinical Therapist Home Base Assertive Community Treatment Team (ACTT) will provide outpatient psychotherapy either at The Providence Center or in the community, on a multidisciplinary ACT team.
Duties and Responsibilities:
Perform, crisis intervention, counseling, screening, client education, referral, treatment planning, and consultation for clients in the program.
Perform a comprehensive psychosocial assessment of assigned consumers.
Perform and record mental status examination.
Assess clinical emergencies, including suicide and danger to others.
Collect pertinent data from appropriate collateral sources in accordance with confidentiality guidelines.
Maintain confidentiality in accordance with TPC policy and legal requirements.
Develop and present, verbally and in writing, a clear clinical formulation based on behavioral data and relevant theory while incorporating psychosocial and family issues.
Provide individual, group, and family mental health and/or substance abuse therapy.
Develop and implement a plan of care with ongoing client input.
Assess clinical emergencies, including suicide and danger to others.
