Job Description
Johnson County Developmental Supports (JCDS) is currently seeking a Service Coordinator. JCDS serves people with intellectual and developmental disabilities. We focus on individuals' abilities, provide choice driven supports, and advocate alongside people to live and work in our community. We aspire to build a community that promotes, supports, and embraces people of all abilities.
Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders.
Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long term individual needs; encourages the establishment and maintenance of support systems; consults and collaborates with other providers in the development, implementation and review of plans.
Provides a variety of services for a designated case load of individuals with multi-faceted and/or ongoing needs; ensures services provided are consistent with individual plans; initiates and attends individual and/or group meetings and provides services in a community based setting; provides transportation of individuals according to the needs identified in plans; organizes and facilitates skill development and support groups.
Completes and submits documentation regarding services provided; evaluates and documents individuals' progress according to measurable goals; reviews quality and effectiveness of services provided.
Job Requirements
Minimum Job Requirements:
Bachelor's degree in Human Services or related field and one year of related work experience are required. May require KS/MO driver's license, depending on department or agency.
Johnson County Government requires reference/background screening for all positions. Specified criteria may vary by Department/Agency.
Education/Experience Substitutions:
Experience may be substituted for degree. Education may be substituted for experience.

The Clinician II is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. Clinician II will be responsible for patient caseload and will provide group, individual, and family therapy. Clinician II will conduct psychosocial and bio-psychosocial assessments. Clinician II will collaborate with the Treatment Team to formulate the Master Treatment Plan, and assist in coordinating discharge planning. The Clinician II has primary responsibility for maintaining communication with the family, referral source and other service providers as needed.
Core Job Duties
Provides excellent individual, family and group therapy to assigned patients following program curriculum provided by Clinical Director.
Completes all required documentation ensuring compliance with all company policies and procedures, state regulations, Joint Commission standards and Medical Needs Criteria. This includes:
- Biopsychosocial Assessment and Integrated Summary (72 hours)
- Comprehensive Problem List
- Integrated Master Treatment Plan and Updates (24 -72 hours based off payer)
- ASAMs (24 hours, based on state standards)
- Individual, family and group therapy session notes (24 hours)
- Discharge Summary (72 hours)
Uses therapeutic interventions to assist patients with accomplishing all treatment plans goals and objectives and successfully complete treatment.
Assesses patient for substance use disorders, risk to self or others and co-occurring mental health disorders.
Manages aftercare recommendations and support that will prepare patient for maintaining successful recovery.
Provides caseload management as per state requirements.
Participates in crisis intervention with patients as needed.
Participates with multidisciplinary teams such as Financial Case Managers to provide appropriate aftercare planning and securing resources.
Completes documentation daily to meet metrics of 92% completion within compliance.
Other Required Tasks:
Attend Daily Flash Meetings in the absence of the Clinical Director or Assistant Clinical Director.
Maintains positive relationships with all staff.
Participates in multidisciplinary treatment plan reviews.
Maintains healthy boundaries with patients and staff.

They will help these high utilizers navigate the behavioral health, social, and justice systems, eliminate barriers, and enhance the effectiveness of partner agencies. This position will be based in Olathe Police Department. The work schedule is Monday through Friday 9 a.m. to 5 p.m., however there is a need for flexibility to work some evenings to meet the needs to the people we're serving.

Job Description:
* Completing broad scope assessment of needs and service gaps for OPTIONS program participants with the OPTIONS team, including clients with mental health diagnoses, intellectual and developmental disabilities, substance use disorders, dementia, traumatic brain injuries, etc.
* Creating broad scope intervention plans for OPTIONS program participants with the OPTIONS team to address the participant's needs, engage community providers, and reduce or eliminate the participant's usage of emergency services.
* Responding in person to 911 behavioral health calls alongside the OPTIONS officer to conduct an assessment to recommend appropriate level of care, including assessing individual risk for suicide and other safety related concerns, and determining the need for resource referrals.
* Completing in person or phone outreach to support individual's post-crisis with connecting to necessary services; this could also be after other 911 calls, like domestic violence, fatalities or traumatic experiences.
* Working with the OPTIONS officer and case manager to carry a crisis management caseload; connecting individuals who have multi-faceted needs to ongoing supports and services.
* Assessing individuals' circumstances to identify needs/or determine service eligibility
* Reviews police reports and determines appropriate next steps, including follow up needs after the initial call to ensure the crisis has resolved and/or if additional resources are needed. Follow up is conducted by phone or an in-person outreach alongside a law enforcement officer.
* Collaborating with other community agencies to coordinate resources and services for the client's benefit.
* Submits clinical documentation regarding services provided and makes necessary referrals to internal and external resources.
Job Requirements
Minimum Job Requirements:
Master's degree in Social Work, Counseling, Psychology, or other Human Services field and Kansas Behavioral Sciences licensure* at the master's level in the area of focus are required. May require a KS/MO driver's license, depending on department or agency.
*Applicants who meet Kansas licensure requirements may be considered if Kansas licensure is obtained by time of an employment offer and licensure is pending confirmation at the time of employment application. Candidates must be able to meet REJIS/KCJIS requirements (for law enforcement positions) and will be required to successfully complete a background investigation, including post-offer polygraph and drug screen, as a condition of employment.

The Clinician I is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. Provides excellent individual, family, and group therapy to assigned patients following program curriculum provided by Clinical Director.

Uses substance abuse and co-occurring therapeutic interventions and de-escalation techniques to assist patients with accomplishing all treatment plan goals and objectives and successfully complete treatment.
Assesses patient for substance use disorders, risk to self or others, and co-occurring mental health disorders.
Uses therapeutic interventions to assist patients with accomplishing all treatment plan goals and objectives and successfully complete treatment.
Manages aftercare recommendations and support that will prepare the patient for maintaining successful recovery.
Provides group therapy.
Provides caseload management as per state requirements.
Participates in crisis intervention with patients as needed.
Participates with multidisciplinary teams such as Financial Case Managers to provide appropriate aftercare planning and securing resources.
Completes documentation daily to meet metrics of 92% completion within compliance.
