Job Summary The Intake and Assessment Specialist provides comprehensive intake and assessment case management services to individuals who are currently incarcerated, ensuring timely identification and response to their needs. Offers expert consultation to staff on prison-related issues and acts as a primary liaison with Prison Health Services, YesCare, the Defenders Association, court officers, and probation and parole offices to promote coordinated care and clear, effective communication. Essential Functions Receive and process referrals from multiple sources for services to incarcerated individuals. Conduct in-person needs assessments to identify client priorities and service requirements. Develop and document individualized service care plans in collaboration with each client. Maintain accurate, timely, and complete activity logs and case records. Collect updated medical and demographic information and obtain informed consent from each client. Manage a reduced caseload, providing targeted case management and advocacy for HIV-positive individuals who are incarcerated or recently released. Responsibilities include visiting clients in prison, at home, in the hospital, and during office appointments to ensure continuity of care and support. Complete all required paperwork, documentation, and referrals within the time frames specified in the Direct Services Policy and Procedure Manual. Record all client contacts for assigned cases in accordance with established documentation timelines. Finalize intake records and information/referral inquiries within 24-48 hours of receipt. Attend weekly one-on-one supervisory meetings to review cases, progress, and professional development goals. Participate in monthly team supervision sessions to share insights and strengthen collaborative practices. Participate in Action Wellness staff meetings and scheduled in-service training sessions. Engage in required on-site and off-site trainings and staff development activities to enhance skills and maintain best practices. Education and Experience Bachelor's degree in Social Work, Human Services, Psychology, or a related field is required. Minimum of two (2) years of experience in case management, social services, or a related field, preferably working with individuals impacted by HIV/AIDS, incarceration, substance use, or mental health challenges. Familiarity with correctional environments and reentry services strongly preferred.
