The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in an independent living facility. Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat in the most functional environment which is the patient's home. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. They implement individualized therapy programs designed to restore, reinforce, and enhance performance. In addition, the Physical Therapist instructs, educates and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. This position provides a flexible schedule and work environment in various settings. RESPONSIBILITIES/ACCOUNTABILITIES:
1. As necessary or applicable, supervision of Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards. 2. Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings. 3. Attends and contributes to patient care, staffing conferences and other related meetings. 4. Makes referrals to other disciplines and agencies in order to address the comprehensive needs of the patient. 5. Consults with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient. 6. Adapts program and/or plan of care according to the needs of the individual patient. 7. Promotes a safe environment and enforces the elimination of fire and safety hazards. 8. Orders supplies and equipment as necessary. 9. Cleans, maintains, identifies, and reports the need for repair of equipment as necessary. 10. Puts patient service first: ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights. 11.Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services. Services include but are not limited to application of therapeutic techniques in areas such as: * Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training

Instructs patient in the skills and techniques required for participation in mobility activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential. 15. Performs routine reexamination as needed/required to modify/progress plan of treatment. 16. Incorporates health and wellness programming into treatment plans for patients. 17. Performs home and community mobility assessments and provides recommendations for safe mobility in the home and community space. 18. Performs other related duties as required. SHAREPTHV1. Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility; and 2. They must have a Master's degree in Physical Therapy; or 3. They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or 4. They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. 5. This program is a continually funded grant through Department of Criminal Justice Services. The Jail Mental Health Program is a partnership involving the Richmond City Justice Center, RBHA and Opportunity.Alliance.Reentry (OAR) to provide mental health and reentry services to qualified residents diagnosed with mental illness in the jail. The Jail Mental Health Program (JMHP) is designed to help reduce recidivism in the Richmond community. Incarcerated individuals can experience difficulty in receiving appropriate mental health care while incarcerated and are at higher risk for recidivating once they are released in the community. The JMHP focus on two components: providing robust mental health care while incarcerated and re-entry services via the wraparound model once they are released.
Services include biopsychosocial and diagnostic assessments, individual and group therapy, implementation and use of the multiple evidenced-based practices, and psycho-education groups. The work location is the Richmond City Justice Center. Hours are Monday through Friday 8-4:30, following the Richmond City Sheriffs Office holiday schedule. The successful candidate will perform difficult professional work providing direct clinical service to clients involving intake, assessing client needs, developing treatment plans, providing prevention services, managing cases, providing individual and group therapy, maintaining records and files, preparing reports, and related work as apparent or assigned. Work is performed under the supervision of the assigned supervisor at RBHA and RCJC.

Documents and charts information about client assessment, diagnostic studies, individual service plans, progress notes, quarterly and annual reviews, discharge summaries and other client related data, correspondence, medical records, etc; prepares and submits grant documentation.
Monitors and audits clinical and medical records and practices for compliance with regulatory, reimbursement and agency standards; reviews documentation for accuracy; assists with reviewing division policies and procedures.
Represents clients and Authority in court and legal proceedings.
Provides transportation assistance.
Provides child care assistance to clients attending treatment groups.

Education and Experience

This position is responsible for the direct provision of emergency, counseling, and intensive case management/coordination services as part of the Region 4 Crisis Response & Stabilization Team (CReST). Work is performed under the limited supervision of the CReST Supervisor and/or CReST Program Manager.
Crisis Response & Stabilization Team (CReST) is a resource for the residents of Region 4 who are in a mental health crisis and need help. With quick response and with assistance connecting to ongoing services, CReST hopes to reduce the cycles of crises and prevent the need for acute psychiatric hospitalizations.
CReST provides a therapeutic response to crisis and is staffed with LMHP-E/LMHP Clinicians providing short term crisis counseling. CReST serves across the lifespan and regardless of insurance. New clinicians will have the opportunity to specialize in adult, children and/or cross-population teams.
Hybrid telework and flexible schedules, with a primiary site location at one of the Region 4 CSB's.
Overnight positions available: 12 hour shifts, 8a-8p, rotating to include varied weekends/days.
Essential Functions
Directly provides emergency supports and short-term crisis counseling services to adults, children, adolescents, and families presenting with significant emotional and behavioral needs, and in psychiatric distress/crisis, as assigned by the CReST Supervisor and according to the program model.
Determines immediate, short, and long-term service needs of the individual and/or family; conducts thorough evaluation and prepares comprehensive assessment.
Develops and implements crisis response plans for individuals and families served through the CReST program.
Refers and ensures linkage of individual and family to other needed services and community resources.
Coordinates individual care with family and other applicable service providers.
Conducts home visits, and school and other community site visits as required and/or permitted.
Monitors, documents, and reports on individual and family treatment progress.
Prepares, documents in, and fully maintains the electronic health records (EHR) of assigned CReST cases.
Collects and enters required individual consumer data and submits reports on individual consumer progress.
Positively communicates and maintains good working relationships with community partners.
Actively participates in all required trainings and meetings with CReST team members, supervisors, and clinical staff.
Maintains CReST caseload as required.
May serve in the role of a Certified Pre-Screener, when applicable.

Education and Experience

As a Field-based BCBA, you will:
Conduct skills and behavior assessments to deeply understand your clients
Develop comprehensive treatment plans and plan evidence-based interventions
Train, supervise, and support the RBTs working on your child's team
Collaborate with supervisors and the quality assurance team to achieve optimal client outcomes
Coordinate with and train caregivers to empower them as partners in their child's progress
Qualifications:
About You:
You need a Master's Degree in Applied Behavior Analysis or related field.
You need to be a Board Certified Behavior Analyst (BCBA) with experience supervising the implementation of ABA-based treatment programs by RBTs.
You need to be licensed in the state of Maryland or actively pursuing licensure at the time of hire.
You need to be dedicated to providing high-quality, outcomes-based care to our clients and their families.

Summary
The Inpatient Case Manager, in collaboration with the clinical team and medical provider, provides discharge planning coordination and intervention. The Care Manager strives to promote patient wellness, improved care outcomes, efficient utilization of health services and minimize denials of payment among a patient population with complex health needs. Essential Functions:
Assessment: Performs initial and ongoing Care Management assessment to determine, based on patient's condition and presentation, care coordination and discharge planning needs as appropriate.
Planning: Creates a focused, anticipated discharge plan of care for assessed high risk patients with identified needs.
Intervention/Evaluation: Collaborates with the clinical team and medical provider to initiate the anticipated discharge plan for patients.
Performance Improvement: Stays abreast of changing organizational, emergency department, Care Management, and clinical trends, regulatory matters and third party payer requirements related to clinical care, discharge planning, and precertification or aftercare benefits.
Requirements/Qualifications:

Essential Functions
Assesses client service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment.
Develops an individual service plan of care and services appropriate to the evaluation with the client/family consensus; reviews and updates service plans.
Makes referrals and linkages to appropriate agencies for services; coordinates client services and treatment with multiple service providers and agencies.
Evaluates the quality of services provided and changes in clients condition and counsels clients as necessary; evaluates clients environment for safety, security, negative factors and productivity; compiles and analyzes data relating to complaints; identifies and works to resolve problems.

Position Requirements

Education and Experience

Registration with the Virginia Board of Counseling as QMHP C (Child) preferred.
Registration as a QMHP-T (Trainee) required. A Qualified Mental Health Professional - Trainee (QMHP-T), is defined as a person receiving supervised training in order to qualify as a QMHP-A/C in accordance with 18VAC115-80 and who has met the minimum requirements and is registered with the Virginia Board of Counseling.
Masters degree preferred.
One year of experience working with individuals with serious mental illness or intellectual disabilities in a behavioral health setting.
Applicant must have valid Virginia driver's license.
Qualifying case management staff will be eligible for a $3K sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.
