Assess the counseling needs of the patient. Assess the patient for substance abuse diagnosis using DSM criteria in determining appropriate level of care based on ASAM criteria. Obtain previous records that are relevant to the current treatment episode. Prepare the substance abuse counseling portion of the patient treatment plan. Provide substance abuse counseling services specified in the patient treatment plan. Develop the patient discharge/continuum of care plan to ensure movement to the appropriate levels of care. Participate fully as a member of a multidisciplinary team for assigned patients. Document all counseling and education services, assessments, reassessments, referrals, and follow-up in the patient's clinical record. Maintain patient caseload (including all case management and utilization review updates, treatment planning duties, and clinical charting as required) Provide coordination of support services for the patient and their family and record the services provided in the clinical record. Reassess patients and revise the substance use counseling portion of the patient treatment plan, as necessary. Facilitate family therapy as assigned. Contact and coordination with referral sources (i.e., legal, school, employer, etc.). Facilitate intervention for crisis-orientated issues utilizing a multi-disciplinary team approach for assigned patients. Carry out administrative and clinical functions as requested. Job Description Hours: full-time, 37.5 hours per week Lynchburg Secondary Compensation is dependent on experience The Licensed Clinician provides assessment, counseling, and care coordination for individuals with substance use and co-occurring needs served through the Appomattox County SUD grant. This role delivers evidence-based treatment, ensures timely documentation, collaborates with a multidisciplinary team, and maintains all required training and competencies. Based in Appomattox County, the position also involves building and sustaining stakeholder relationships with referral sources. Depending on licensure and agency needs, responsibilities may include clinical supervision of residents and/or QMHP-T staff, as well as supporting flexible scheduling to meet client demand. A valid Virginia driver's license and adherence to agency standards of ethics, customer service, and teamwork are required. Required Education and Experience Minimum Required Education Master's Degree from a program that prepares individuals to practice counseling (60-hr Master's Program), which is offered by a college or university accredited by a regional accrediting agency.
