Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required

About the role

What you will need

PHYSICAL WORK ENVIRONMENT:
Work is typically performed in an office setting. Inventory management requires some lifting and moving of objects up to 30lbs. Excellent communication skills are essential. This includes the ability to speak clearly and fully comprehend written and spoken English and will include communication with people for whom English is not their first language. COMPENSATION:

The Edward Thomas House Medical Respite is a unique medical harm reduction program that provides shelter care with embedded nursing, mental health, and case management services to unhoused individuals. **PRIMARY JOB RESPONSIBILITIES**
+ Contribute to and document initial and ongoing bio-psychosocial patient evaluations and assessments.

+ Facilitate therapeutic sessions and patient/family conferences.
+ Deescalate potentially dangerous situations and promote stability on unit.
+ Recognize and communicate observations related to effects and side effects of medications and signs of substance intoxication or withdrawal to RNs.

What You'll Do
As a Child & Family Therapist, you will:
Provide individualized, strengths-based clinical services in home, school, community, and office settings.
Collaborate with a multidisciplinary team to support youth and their caregivers.
Deliver evidence-based therapy using creative, flexible, and culturally sensitive approaches.
Receive training in intensive mental health intervention and WISe model integration.
What You Bring
MA/MS/MSW in Behavioral Science-related field
Meets criteria as a Mental Health Professional (MHP) per WAC
Preferred: Meets requirements for Child Mental Health Specialist
Must pass a criminal background check
Valid WA Driver's License, reliable vehicle, and proof of insurance
Must submit Agency Affiliated Counselor application if not already licensed
Preferred Qualifications
Experience in medical, mental health, or social services fields
Knowledge of or interest in Evidence-Based Practices
Strong collaboration, problem-solving, and communication skills
