Major Responsibilities:
Conducts a physical therapy evaluation of each patient and establishes a plan of care and treatment goals. Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations utilizing appropriate methods. Evaluates outcomes within an appropriate time frame.
Provides on-going assessment of patient response to treatments and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with status report on a regular basis.
Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Completes a plan of care as indicated and provides physician with status report. Maintains and updates clinical and patient records according to state and federal guidelines.
Manages assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages patient's medical condition including medications. Acts as a patient advocate to insure optimal outcomes. Manages assigned caseload for optimal functional and financial outcomes; reviews caseload for Low Utilization Payment Adjustments (LUPA), optimal number of visits based on progress, Prospective Payment Plan (PPS), and Outcome Based Quality Initiatives (OBQI) outcomes.

Performs Home Health Aide joint visits as appropriate to maximize patient's functional abilities in the home. Teaches appropriate treatment procedures and supportive activities to patients and caregivers and develops plan for obtaining adaptive devices as needed.

Licensure, Registration, and/or Certification Required:

We are seeking Crisis Counselors to join our team. This full-time position is responsible for providing 24-hour counseling services for consumers in need of emergency mental health services. Services include assessment of consumer functioning and implementation of appropriate short term treatment plan developed in tandem with consumer and designed to meet consumer short term physical and mental health needs. The Crisis Counselor works cooperatively with other community professionals to maintain consumer safety through assessments for general mental health, suicide, homicide, and ability to care for self.
Key Responsibilities
Serve as a liaison between the consumer and other service organizations in providing and receiving referrals.
Provide 24-hour crisis intervention services.
Assess consumer mental health status and coordinate emergency services.
To assess for and implement Chapter 55 Protective Placements and subsequent placement at appropriate facilities when necessary after normal business hours for county Adult Protective Services.
Qualifications
Education:
Required: Bachelor's degree or equivalent work experience
Extensive training is provided to new hires so new graduates are encouraged to apply.
Skills and Competencies:
A working knowledge of human growth and development, mental health, adolescent psychology, marital and family structure, and the impact of the environment on individual behavior.
Time management skills and the ability to meet work demands on a moderately independent basis.
Ability to establish and maintain relationships within and outside the agency in a professional manner.
Ability to evaluate one's own performance, capabilities, and limitations with reasonable accuracy.
Valid Wisconsin Driver's License with reliable personal vehicle. Maintain required auto insurance and acceptable driving record requirements.
