Develops and maintains positive collaboration with other social service agencies serving clients. Maintains knowledge and familiarity with referral sources within the community. Provides consultation when needed.
Has no unexcused absences from any unit, department, and mandatory Agency meetings and training.
Maintains license or registration appropriate to profession and appropriate to job requirement, including appropriate amount of continuing education at all times of employment.
Maintains agency-required annual training including client rights and grievances, safety training, infection control, and others as required by OHMHAS.
Remains free from citations by other providers or by consumers for unethical practice and meets the standard for their own profession.
Adheres to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.
Participate in at least 6 hours of supervision for every 3-month period or meet the requirements of supervision required by license/registration (whichever is greater).

Qualifications
Minimum Qualifications:
Education: Associate degree in human/social service field, or High School Diploma with equivalent field experience
Experience: 3 years working with SMD population
Certification: First Aid; CPR; De-escalation.

The Community Family Intervention provider, as a member of the Outpatient Family Services Team is responsible for providing intervention services to children and families. These services may be provided in various locations both internal and external to the organization and is driven by the clinical needs of the entire family unit. Transport clients and family members occasionally on an as-needed basis.
Maintain accurate documentation of direct and indirect services provided in accordance with ODMH, ADAMH, and agency standards.
Gather statistical and research information to be used for outcome purposes.
Participate in staff and team meetings for the schools and agency, staff development and in-service training, planning interventions and regular supervisory conferences.
Bilingual (English/ Spanish). Fluent in both spoken and written communication for both languages.
Knowledgeable of community resources that can support individuals in the Hispanic/ Latino community.
Able to demonstrate cultural competency as it relates to working with individuals from different Hispanic or Latino cultural backgrounds.

EDUCATION & EXPERIENCE
Bachelor's degree in social work, Counseling, or other related discipline is required.

Rendering Provider number required based on level of licensure.

Make corrections to errors on any administrative or UM reports including but not limited to incorrect delivery methods, blank notes, overlapping notes, invalid diagnoses, and invalid/incorrect place of service.
Completes paperwork within defined timelines (Progress notes should be documented on the same day services are provided but are due - at the latest - within 7 days of the service that was provided).
Make contact with newly assigned cases within 48 hours of assignment and have first appointment within 7 days of case assignment.

Qualifications
Who you are:
You have a bachelor's degree in social work, or related field; master's degree preferred.
You have an active LPC, LSW, or MFT licensure, or are license exam eligible.
You have an active driver's license and proof of auto insurance.
You preferably have previous work experience as a Counselor, Therapist, Social Worker, or Clinician.

This part-time role is up to 20 hours per week, and designed to provide direct care and other related technical and clinical support activities that align with Empowering to Elevate Mission, Vision & Values.
Reports To: Clinical Director or Clinical Supervisor
Essential Tasks, Duties, and Responsibilities:
Responsible for the clinical management of his/her assigned clients.
Assists in identification of treatment/contract goals, completing the initial psychosocial assessment.
Responsible for maintaining contact with the assigned client's referring/outpatient therapist during the course of the clients stay and the family when/if appropriate.
Completes and updates treatment plan for assigned clients.
Meets with clients frequencies according to their treatment plans, to help explore identified issues and behaviors in relationship to current symptoms.
Leads and participates in group meetings, meals and other activities in the milieu.
Provides comprehensive understanding of current behaviors and relational dynamics while using a variety of therapeutic modalities.
Attends the weekly team meetings and share observations, comments and important information about clients with the treatment team and provides input into the development of the weekly schedules for all of his/her assigned clients.
Responsible for proper charting and ensuring that all documentation is completed in an accurate and timely manner.
Assists in meal/snack supervision as assigned and attends special occasion activities for the clients.
Assists with discharge planning, including a written discharge summary and obtaining referral sources and necessary support systems once the client is discharged.
Depending on credentials and/or licensure, may serve as a preceptor and/or clinical supervisor to graduate students while they are completing an internship at Empowering to Elevate.
Updates job knowledge by participating in educational opportunities; reading professional publications related to Empowering to Elevate Mission and population.
Perform other duties as required.
Knowledge, Skills, and Abilities:
Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).
Bachelors or Masters Degree in Social Work, Counseling; independent licensure preferred; two years previous experience preferred.
Valid Ohio Drivers License and maintains a driving record that allows that individual to be insurable with the insurance company providing Empowering to Elevate with vehicle insurance. Willingness to travel for various reasons, mainly during the day but occasionally overnight.
Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
Physical Demands:
Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
Must be able to lift 20 pounds independently.
Must be able to remain alert to problems, needs or emergencies that might arise on any shift, maintaining a safe environment for the clients.
Ability to communicate (verbal and written) with all levels or personnel, internal and external to the company.
