Qualifications:
Active, unrestricted Physical Therapist (PT) license in the state of assignment
Current BLS certification (AHA)1+ year of recent home health experience (within the last 2 years)
Experience conducting PT evaluations and developing therapy plans
Familiar with Medicare-certified home health guidelines and OASIS-E documentation
Experience with EMRs such as Homecare Homebase, Net
Smart, or Epic
Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
Reliable transportation with valid auto insurance and ability to travel between patient homes
Home Health Physical Therapist Contract Details:
$2,000 - $2,166 per week
13-week contract
Paid by the hour - not by the visit5-6 visits per day (45-90 minutes each, based on complexity)
Travel time factored into productivity expectations
Key Responsibilities:
Perform initial and ongoing PT assessments and develop individualized plans of care
Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
Conduct gait and balance evaluations, home safety assessments, and mobility training
Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
Document all care in compliance with Medicare, OASIS-E, and agency standards

* Provide safety leadership for all departmental employees, work with area personnel to develop and communicate safety expectations, and establish a culture of safety accountability.
* Manage overall day-to-day operations of 1 Recovery Boiler, Evaporator/Concentrator area, 2 Power Boilers, 1 Package Boiler, 2 Steam Turbines, 2 Air Compressors, 3 Hydro Turbines.
* Ensure 100% environmental compliance.
* Align Recovery area operations with pulp mill to ensure both operations function smoothly and manage process flow in an orderly and efficient manner.
* Plan and coordinate all maintenance efforts and reliability programs for Recovery, Power Boilers, Steam Turbine, Hydro Plant, and Air Compressors, and provide project management and oversight for annual maintenance outages.
* Work closely with maintenance to develop, implement, and maintain process reliability initiatives and enhance operator-based equipment monitoring and basic care functions.
* Ensure all area personnel are properly trained in process operations and required drills to manage upset or emergency conditions.
* Provide leadership to area personnel in continuous improvement with a focus on improving operational performance.
* Manage overall cost of operations with a focus on reducing cost without negative impact to efficiency, employee safety, or environmental compliance.
* Manage usage of auxiliary fuels in Recovery and Power Boiler operations with a focus on minimizing costs.
