The Direct Care Worker is responsible for providing a therapeutic atmosphere within the dorm or group home setting. The Direct Care Worker will monitor and interact with our kids during meals, while they do their chores and homework, during free time and at bedtime providing a consistent and warm environment that they desperately need. SAFY is seeking a full-time Case Manager I for our High Fidelity Wraparound program in Louisville, Kentucky. As a Case Manager, you will be responsible for providing in-home support services for families whose children are at risk of removal. This position will be based in Louisville but will be mainly working in the field. The State service region is Salt River Trail with the primary county being Franklin. High Fidelity Wraparound (Wraparound) is an evidence-based approach rated as "Promising" on the Title IV-E Prevention Clearinghouse. Wraparound centers on individualized care planning and coordination to support children and families with complex needs.
