About the Memory Care Engagement Director: The Memory Care Engagement Director is responsible for leading, coordinating, and enhancing person-centered care services in the memory care unit. This role serves as the primary non-clinical point person, ensuring residents with Alzheimer's disease and related dementias receive person-centered, engaging, and dignified care. The position develops individualized service plans (ISPs), completes Uniform Assessment Instruments (UAIs), oversees therapeutic engagement programming, and serves as the main liaison for residents, families, and staff. This is a full-time, 32-hour per week position. Monday through Friday, plus an occasional weekend.
Qualifications of the Memory Care Engagement Director:
Five years' experience in the application of therapeutic, recreational, and leisure activities in a geriatric environment with a focus on dementia care, preferred.
Bachelor's Degree in an activities or health services-related field preferred. ADC or CTRS required.
Valid Virginia Driver's License; CPR and First Aid certified

Responsibilities of the Memory Care Engagement Director:
Plans, organizes, evaluates, and delivers person-centered therapeutic activity programs designed to meet the highest practical level, utilizing all dimensions of wellness.
Collaborates with the Life Enrichment Manager in planning and execution of resident activities with a primary focus of promoting person-centered programming based on the individual's physical and cognitive abilities
Completes UAI's and ISPs with timely updates to reflect residents' needs and collaborates with the interdisciplinary team. Ensures compliance with Virginia DSS regulations, licensing standards, and community policies.
Conducts initial activities assessments for admissions to care areas with a focus on the Resident's life history and activity preferences.

About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
Administer and order point of care and lab-based testing as needed
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities

During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. Administer and order point of care and lab-based testing as needed

Continue to lead the Interventional Radiology Technologist Training Program which identifies, selects and trains candidates to fulfill the duties of an interventional radiology technologist.
Conducts educational assessments to identify learning needs using standards of specialty practices, clinical pathways and guidance from Interventional Radiology Technologist professional association guidelines.
Implements Inova Health System programs for team members to achieve patient care and outcome standards, such as core measures, and patient satisfaction.

Education: Bachelor's degree; American Registered Radiologic Technologist (AART), graduate of an accredited school of Radiography
