Primary Duties (Essential Functions): · Assist with coordinating and support of residents and their families with transitions to a higher level of care. (ALF/PC, SNF). · Collaborate with the appropriate entities to coordinate services for residents being discharged from the hospital. Ensure residents are referred as appropriate for supportive services. · Act as a liaison and advocate for residents and their immediate family members and/or resident representative. Communicates in a timely basis with the resident, family/responsible party and appropriate internal staff on changes in social, emotional, psychological, or overall health status of individual residents. · Track the progress of Independent Living residents, identify those who may benefit from additional services, and facilitate and host Life Care educational meetings. · Participate in general education sessions for residents to highlight the role and value of case management. · Assist in the assessment process for new residents and annual wellness assessments for established residents when needed. · Evaluate current independent living residents as needed to determine if they meet appropriate level of care, per established criteria. · Evaluate the need for supportive services for current residents. · Lead the monthly Level of Care meeting, which include preparing an agenda, maintaining records of meetings, and actions taken/recommended. · Assist Community staff in discussing resident's needs with Resident and or Resident Representative · Review all IL security reports daily to monitor of the need of supportive services or a potential decline in level of care · Do bi -annual observations of all IL residents in attempt to identify the need for supportive services or change in level of care · Provide education on a quarterly bases to IL residents on levels of care and services offered on campus NOTE: This is not to be considered as an exhaustive statement of duties, responsibilities or requirements and does not limit the assignment of additional duties for this position.
