Essential Duties and Responsibilities: Serves as a Staff Development Educator to ensure that educational needs are met throughout Masonicare Health Center. Demonstrates advanced clinical knowledge in geriatric nursing through direct and indirect clinical practice. Coordinates all educational training programs for licensed and unlicensed staff. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops. Provides an annual calendar of educational activities for staff including learning needs identified by nursing leadership. Identifies essential competencies for licensed and non-licensed staff within the Nursing Department and ensures appropriate educational opportunities exist to promote the growth and the development of staff. Plans, delivers, and evaluates educational programs for licensed and non-licensed staff. Coordinates the Student Nursing Program with area Board of Education and local universities in accordance with program guidelines and contracts. Develops and delivers clinical orientation program for licensed and non-licensed staff within the Nursing Department. Utilizes Relias Learning Management System to track employee education. Manages Clinical Preceptor education program, ensuring on-going support, resources and educational tools are delivered as needed. Maintains staff program attendance records in accordance with the State of Connecticut DPH and CMS requirements. Provide and maintain employee education record for CPR/BLS training for direct care staff as required. Performs all job functions in line with Masonicare mission, vision and values. Reports all unsafe conditions and promptly reports an injury or exposure identified. Performs other related duties as assigned or requested. Qualifications Education: BSN strongly preferred Experience: At least three years experience as a nursing educator in a geriatric setting. Excellent human relations and oral/written communication skills. Working knowledge of Relias Learning Management system desired. IV Therapy certification preferred. Administers notices of non-coverage to Medicare patients who do not meet medical necessity. *Performs comprehensive assessments and reassessments of the patient's condition and discharge planning needs including the psychosocial, physical, educational, cultural aspects, and social determinants of health. Works with the clinical team to develop and modify the care plan to meet the needs of the patient. *Collaborates with other disciplines in patient evaluation and treatment and initiates referrals appropriately. Acts as the principal communicator to the patient, families and external case managers regarding insurance benefits / reimbursement, treatment plan, progress, length of stay and discharge plan. *Initiates discharge planning in a timely fashion, develops and revises individualized discharge plans as indicated by assessment and patient response to treatment. Evaluates overall plan for effectiveness; involves the patient and family in formulation of goals. *Meets monthly goals for length of stay management. Maintains compliance with The Important Message from Medicare, transfer notes, post-discharge phone calls, and other functions subject to audit. *Portrays a professional demeanor reflective of the ICARE values to both internal and external stakeholders at all times. *Demonstrates an understanding of level of care criteria and reimbursement factors for home care, rehabilitation, residential treatment and long term care in development of discharge plans. Seeks alternatives to facilitate discharge planning. *Provides patients and families support and information about their current disability. *Provides education and support to hospital staff regarding community resources, managed care issues, or payment / payer issues. *Demonstrates an ability to be flexible, organized and functional under stressful situations. Utilizes critical thinking skills and sound judgment in priority setting and delegation. *Practices autonomously, consistent with evidenced-based standards.     Masonicare Health Center - Wallingford, CT Night Shift / 32hrs/wk / EOW 50 bed Behavioral Health Hospital Unit Coordinates the daily clinical operations of the unit and directs the delivery of patient care. Oversees the integrated care plan to insure it provides holistic, medical, spiritual and emotional care, encompassing quality, dignity, compassion and confidentiality for each individual. Upholds the standards of nursing practice to achieve optimal clinical outcomes and insures the team utilizes quality customer service principles in their interactions. Essential Duties and Responsibilities: Leadership: Acts as role model for staff by promoting professional practice through effective team building, communication, and advocacy. Identifies opportunities for workflow/operational improvement in patient care planning and care delivery and presents to Director, Acute and Psychiatric Services using evidenced-based literature to support case for improvement. Demonstrates and promotes critical thinking skills and effective nursing judgment in patient interactions and role models behaviors for staff. Manages assignment process on a daily basis including: Delegate assignments to appropriate staff on all shifts; Completes patient assignment sheet taking into account patient acuity, staff level, ratio, ability, and medical schedule assignments; Makes appropriate change to assignments as patient/unit needs/acuity changes; Communicates changes in level of care/acuity to staff; Ensures that tasks are carried out as assigned; ensures that all staff arrive to work on time and depart on time; reports variances in a timely fashion, to the Director/and or Supervisor on the off shifts. Acts as liaison between interdisciplinary team members on a daily basis to ensure that behavioral care plans are developed and appropriate to meet the needs of the patients and the nursing care plan is updated and appropriate Attends Team meetings and participates as the clinical representative facilitating the development of behavioral interventions for patients as needed and communicates plans to clinical staff.Includes: Shares appropriate patient information and concerns; Documents and updates treatment plans per unit policy; Coordinates scheduled discharges with the team, and reports plan to assigned staff for preparation; Communicates 1:1 status every shift and if able to D/C 1:1 goal no more than 3 on 1:1 at a time. Participates in the change of shift report for 1st shift (7 am to 3 pm) and 2ndshift (3 pm to 11 pm) and ensures:a timely start to shift report; All staff members have received report, including CNAs and staff on 1:1 assignments; All nursing staff have taped shift report. Rounds with unit staff daily and throughout the shift to assess unit activity with regard to patient flow (admissions, discharges), changes in patient acuity, troubleshoot throughput issues, and mediate other issues/ concerns. Takes lead on incident reporting and ensures that all required documentation and notifications are complete prior to final submission. Updates care plans and patient profiles as patient conditions change. Ensures that inter-shift environmental rounds are completed. Ensures that all immediate concerns related to patient care, staff, the environment, IT, etc. are directly reported to the Director. Manage staffing needs to ensure unit has adequate staffing for shift.Responsible for reaching out to staff to cover callouts and existing holes. Will participate in a rotating on-call schedule to address any immediate needs or crisis related to patient care, environment, staffing, etc. Minimum Qualifications: Education: Graduate of school of professional nursing, BSN required or working towards Experience: Two years acute care clinical experience, with knowledge in financial, regulatory and legal aspects of healthcare preferred. Under the direction of the Assistant Director of Nursing and the Director of Nursing, the Nursing Supervisor has the authority and responsibility for carrying out established standards of practices and care and providing direction in the continuing provision of nursing services. Upholds the nursing program during the off shift through the allocation of nursing personnel, coordination of nursing activities, supervision of nursing staff and serves as a clinical resource to all staff. Demonstrate knowledge and skill to provide care based on physical, psycho/social, educational, safety and related criteria appropriate to age, needs and abilities of the resident/patient in his/her assigned unit. Interacts professionally with staff, physicians, patients and families to ensure that care is effectively and efficiently delivered. Communicates effectively to all staff through verbal reports and written documentation and attends necessary administrative meetings to disseminate important information to off shift staff. Enhances professional growth and development through participation in educational programs both in and out of the facility. Attends all education programs, mandatory in servicing and meeting. Performs other duties as assigned. Minimum Qualifications: Education: Graduate of school of professional nursing, BS in nursing or healthcare related field preferred. Experience: 1-2 years of experience in a supervisory capacity preferred. Certificates, Licenses, Registrations: Registration and current RN license in the State of Connecticut; CPR certification Key Competencies: Ability to accurately assess and prioritize resident needs.
