Job Summary
This full time, Day shift position will be a float position for the four facilities within the Spaulding network: Charlestown, Cambridge, Brighton, and the Cape. (SRH, SHC, SRB, SCC).
Summary:
Responsible for completing intake interviews; determining need for medical and psycho-social evaluations; reviewing therapist and physician evaluations, care objectives, and plans; coordinating services being provided; advocating for needed services and entitlements.
Essential Functions:
Complete psychosocial assessments covering psychological, social and environmental concerns, including evaluation of mental status.
Review daily referrals and screenings of clients.
Audit case file on a regular basis for completeness and accuracy.
Assist in discharge planning as required
Serves and protects the hospital community by adhering to professional standards, hospital care policies and procedures, federal, state, and local requirements, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.

Essential Functions
* Provide professional nursing care under the direction of a RN/CM, HPC-RNC or Hospice & Palliative Care Manager.
* Provide skilled nursing visits to patients and families, including informational or evaluation visits, admission visits, re-visits, or discharge visits.
* Participate in the development of the care plan along with other Interdisciplinary Group IDG members.
* Follow the plan of care and carry out interventions established by the R.N. CM.
* Recognize and report changes in patient condition to the RN/CM or CTM as directed.
* Communicate information on a timely basis, consistent with VNA/Hospice policy, and the urgency of the situation.
* Provide care within the scope of clinical practice and level of competency (see competency policy/procedure) including: assessment of patient status, physical, psychosocial, and environmental; medication monitoring and administration; patient/family teaching; emergency intervention.
* Perform skilled nursing services in homes, skilled nursing facilities, hospital, and other community settings.

ECMO, which stands for extracorporeal membrane oxygenation, is an advanced technology that functions as a replacement for a critically ill child's heart and lungs. It's used to support a child who is awaiting surgery, or to give a child's vital organs time to recover from heart surgery or disease. Boston Children's Hospital is home to one of the largest pediatric ECMO programs in the world; we provide services to critically ill children in the New England region, and to those who are referred from other states and internationally. We serve between 50 and 60 patients with severe respiratory or cardiac problems each year. Since its establishment in 1984, the ECMO Program has supported more than 900 children. Throughout the 20-year history of the ECMO Program at Boston Children's, we have gained considerable expertise, minimized complications, and increased its success rate for survival to 60 percent, higher than the national average of around 40 percent.
