The RN in infusion therapy is responsible for ensuring the safe and effective delivery of infusions in various healthcare settings, such as outpatient infusion centers, hospitals, and home care. This position requires excellent technical skills, a strong understanding of infusion therapies, and the ability to manage complex patient care needs. Key Responsibilities: Patient Assessment and Planning: Assess patients' medical history, current health status, and specific needs related to infusion therapy. Evaluate the patient's veins for appropriate IV access, considering factors such as vein condition, prior infusion history, and overall treatment plan. Develop individualized care plans for patients receiving infusion therapy, ensuring treatments align with physician orders and patient needs. Infusion Administration: Administer prescribed infusions, including IV medications, fluids, biologics, blood products, or chemotherapy, ensuring the correct dose, rate, and technique. Insert, maintain, and manage IV access devices such as peripheral IVs, central lines, PICC lines, or ports. Monitor patients during the infusion for signs of adverse reactions, complications, or side effects (e.g., allergic reactions, infiltration, phlebitis). Adjust infusion rates or volumes based on the patient's condition and physician's orders. Patient Monitoring and Safety: Continuously monitor the patient's vital signs, symptoms, and response to therapy throughout the infusion process. Respond promptly to adverse reactions, such as allergic responses, complications at the IV site, or systemic reactions. Take immediate action in emergencies, including stopping the infusion, administering medications (e.g., antihistamines, epinephrine), or initiating life-saving measures as needed. Patient Education: Educate patients and their families about the infusion process, including the purpose of the therapy, potential side effects, and how to recognize complications or adverse reactions. Instruct patients on proper care and maintenance of infusion sites, as well as post-infusion care. Provide guidance on at-home care for patients receiving home infusion therapy, ensuring they understand the importance of follow-up care, signs of infection, and other related instructions. Documentation and Record-Keeping: Maintain accurate and detailed records of the infusion process, including patient assessments, IV access site monitoring, medications administered, and patient responses. Document any complications, adverse reactions, or changes in patient condition in the patient's medical record. Ensure documentation complies with regulatory requirements, including adherence to HIPAA standards. Infection Control and Safety: Follow strict infection control protocols when preparing and administering infusions, using aseptic techniques to prevent infections at the IV site. Monitor for signs of infection at the infusion site, such as redness, swelling, or drainage, and take appropriate action if infection occurs. Ensure that all equipment used for infusion is sterile, and that safety protocols are followed to minimize risks to patients. Collaboration with Healthcare Team: Collaborate with physicians, pharmacists, and other healthcare professionals to develop and adjust infusion therapy plans, ensuring that patient needs are met. Communicate effectively with the multidisciplinary team about any patient concerns, changes in condition, or infusion-related issues. We also treat adults who have developed complications or need further procedures as a result of long-standing or a previously treated congenital heart defect (CHD). Key Responsibilities: Providing care for critically ill cardiac medical & surgical patients of all ages. Working collaboratively to provide family-centered care as a member of a multidisciplinary team that provides expert cardiovascular care. 36 hours/week, Day/Night Rotation with every third weekend Minimum Qualifications Education: BSN required Experience: A minimum of 2 years of critical care RN experience. Mobilizes resources to maximize efficiency of care delivery. PRINCIPAL DUTIES AND RESPONSIBILITIES A. Care Facilitation Coordinates and insures implementation of the plan of care, utilizing case management principles. 1. Prior to or within 24-48 hours of admission the RNCC, by interview of the patient/family, discussion with physician team and/or attending MD and other team members, develops a provisional treatment program and tentative discharge date. 2. Reviews daily treatment plan with physicians, nurses and patient / families to insure interdisciplinary communication and coordination is occurring. 3. Participates with nursing staff and physicians in patient care rounds to contribute to plan of care and monitor and report patient progress. 4. Collaborates with other departments to expedite sequencing and scheduling of interventions, consults, treatments and ancillary services. 5. Provides for daily continuity with patients to assure patient needs related to discharge are met. 6. Incorporates knowledge of utilization management principles and payer contracts into patient plans of care. Keeps physicians and nurses informed of implications. 7. Presents alternatives to inpatient stay to attending MD, team and patient / family based on assessed patient level of care and insurance benefits. 8. Seeks assistance and/or consultation from Care Coordination leadership with plans for outlier and potential or actual resource intensive patients. 9. Interacts with internal and external health care providers to facilitate patient care including post discharge services. 10. Contributes to the development, implementation and monitoring of practice guidelines. 11.
