Carry out treatment protocols that support mobility, strength, balance, and functional recovery. Monitor patient response to treatment and communicate updates to the PT or Case Manager. Maintain accurate, timely documentation in accordance with agency policy and regulatory requirements. Assist with educating patients and caregivers on exercises and mobility techniques. Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
