Key Responsibilities:
Provide individual, group, and family counseling in various settings tailored to client needs.
Complete intake/screening and comprehensive assessments within designated time frames.
Develop and document individualized treatment plans based on assessments.
Participate in clinical meetings, providing feedback on cases.
Maintain accurate documentation aligned with regulatory standards.
Facilitate group counseling and therapy sessions, including trauma-informed care.
Manage crisis situations, offering immediate support and intervention.
Collaborate with internal departments to ensure service authorization.
Represent the agency professionally in the community.
Education, Experience, and/or Credential Qualifications:
Must possess at least a Master's degree in Social Work, Psychology, Counseling, or related field.
Holds licensure as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) in Missouri.
Must be willing to become licensed in additional states as required.
Comfortable providing telehealth services (training will be provided).
Additional Qualifications:
Current driver's license and acceptable driving record.
Knowledge of Collaborative Service Goals (CSGs) and commitment to ongoing training requirements.
Physical Requirements:

Provides culturally sensitive, professional therapeutic services in accordance with professional licensing standards and agency policies.
Conducts highly skilled screenings, assessments, treatment interventions; develops treatment/rehabilitation plans; conducts individual and group counseling; interprets tests and interview results and makes diagnoses; and provides other advanced clinical counseling or social work services for consumers who have complex behavioral problems.

Job Description

Assist in assessing patients to develop and initiate treatment plans based on the evaluation results
Assist in reviewing the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
Provide direct patient care under the supervision of a Physical Therapist, following established protocols and patient care guidelines
Document findings, progress, and instructions to patients and caregivers
Support in ensuring patient care performed by assistants and rehabilitation technicians meets standards
Assist in completing written plans of care, developing goals based on evaluation findings, and contributing to comprehensive programs to attain goals
Support in implementing physical therapy treatment programs and providing essential physical therapy treatments
Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies, under the guidance of a Physical Therapist
Assist in ensuring timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports
Qualifications

Current state of Missouri license and CPR certification
Ability to work efficiently/effectively in an autonomous environment

ESSENTIAL FUNCTIONS OF THE JOB
Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical
Therapist.
Communicate with the Physical Therapist, the patient and/or family and facility staff any pertinent information concerning patient progress, problems and plans.
Participate in Patient Plan of Care Meetings and Interdisciplinary Team Meetings if asked by the Physical Therapist.
Participate in in-service training program for other staff in the facility.
Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedure.
Instruct patients' families and/or nursing staff in maintenance programs under the supervision of the Physical Therapist.
Participate in discharge planning under the supervision of the Physical Therapist.
Achieve or exceed the minimum productivity standard established by CRI.
Recommend any needed equipment to the Physical Therapist which would facilitate improved delivery of clinical services.
Report any problems with department equipment to the appropriate personnel.
Act as clinical trainer for affiliating Physical Therapist Assistant programs for area Universities and Colleges.
Comply with the Physical Therapist Assistant Licensure and Certification Standards.
Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.
Consistently portray the mission, vision, core values, cornerstones and professional image of Concept Rehab, exercise good judgment in the performance of the job.

Current state license to perform massage therapy
Active liability insurance for massage therapy is required
Pass background checks
Capable of effectively performing full-body 50-minute massage services
