Job DescriptionSalary: $25-$28 per hour
Freedom Behavioral of Monroe is seeking a Full Time Therapist to work in our Outpatient Therapy Program. The Licensed Therapist works as an integral part of our social services program which is necessary to meet the legal, organizational, medical staff and regulatory agency guidelines. The Licensed Therapist works with our therapy services, case management, education and discharge planning team for the patient's well-being. All duties are to be done in accordance with Joint Commission, Federal and State regulations, Allegiance and Freedom policies and procedures and Performance Improvement Standards.
Essential Functions:Provides clinical supervision of individual, group and family session for patients as indicated in the supervisor notes. Participates as a member of the multi-disciplinary team and participates in chart reviews for quality assurance. Work with the clinical department including the following functions: discharge and after care planning, completion of discharge summaries, completion of clinical documentation and providing guidance to the clinical staff. Review psycho-social documentation, completion of outcome studies assessments and development clinical notes, Provides clinical counseling services; including group, individual and family services including communication in an age/disability/cultural sensitive milieu. Performs other duties and projects as assigned.
Educational/Experience Requirements/Qualifications/Skills:
Must possess an LPC, LCSW or LMSW, PLPC license in good standing with the State of Louisiana.
Mustbe a (1) LPC in the State of LouisianaOR(2) must possess one of the following:
CSW, LMSW.
Preferred:Three (3) years of full-time experience in psych
Excellent assertive communication skills, good writing and composition skills and able to organize and prioritize high volume workload. Must be able to work under close supervision.
Therapeutic Intervention/De-escalation education.

Under the direction of the licensed/registered physical therapist, optimizes functional abilities and skills of patients in facilities by: assisting in screening and initial evaluation of all patients; implementing treatment for all assigned patients; delivering treatment according to the established treatment plan; accurately documenting progress toward objectives and communicating patient status and needs to the patient, the patient's family, the staff and other professionals.
Provides direct supervision of Rehab Aide in skilled tasks during group and individual treatment in a manner that promotes appropriate utilization of resources and development of clinical skills in accordance with State Practice Acts and Lagniappe Rehabilitation policy.

Responsibilities
The Home Health Physical Therapist Assistant (PTA, LPTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
* Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist.
* Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
* Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
