The Home Health Physical Therapist Assistant (PTA, LPTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist.
Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the physical therapy assistant.
License Requirements
Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
Current CPR certification is required.

The Home Health Patient Care Manager Assistant (Licensed Practical Nurse, LPN) is responsible for assisting the Patient Care Manager in coordinating a team of staff to assure the continuity of high-quality care to home health patients assigned to the team's area.
Ensures that the team is committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines.
Prepares patient list and necessary documents for weekly case conferences.
Provides direct patient care, as necessary, in accordance to scope of practice, plan or care and physician orders, and in coordination with other members of the health care team. May not alter the plan of care.
Assist Patient Care Manager in clinician documentation review/audits to ensure complete, appropriate, and accurate submission of all documentation by field staff.

WHAT YOU DO
You assess each guest needs and inquire about contraindications before beginning services. You maintain cleanliness of the workstation and treatment room throughout shift while disposing of trash and dirty linens and securing supplies and equipment at the end of each shift. The Licensed Massage Therapist (LMT) role at MPOWER Physical Therapy is designed for individuals who value long-term stability, ease of access to referrals, and organic collaboration. LMTs work with physical therapists and personal trainers to provide safe, medically supervised, and collaborative massage therapy to our clients & patients. Part-time LMTs (>20 hours/week) can receive a comprehensive benefits package including health insurance, 401k, paid time off, licensure reimbursement, and more.
MINIMUM ESSENTIAL JOB REQUIREMENTS
Understand, implement, and uphold the MPOWER model
Evaluate, treat clients, and complete documentation in a professional and timely manner
Maintain a set number of massage hours per week set by MPOWER management
Maintain a flexible work schedule to meet the needs of the facility (including availability for all team and individual meetings) and a minimum of 20 weekly hours
Integrate other MPOWER services into client care including physical therapy, personal training, nutrition, and collaboration with these and any other referring providers
Communicate with physical therapists and other referral sources regarding the treatment and progress of their patients and clients
Manage administrative tasks consistently including but not limited to ensuring all client sessions are paid for and checked out and check company email daily
Manage, track, and follow up with clients after each massage to ensure retainment of clients
Keep accurate record of massage hours via MPOWER's administrative software daily; PRN massage therapists will be compensated for massage, room flip, and meeting time only
Play an active role in recruitment and training as needed
Submit accurate and timely tip reporting
Communicate time off at least one month in advance and ensure all clients are scheduled to work with other massage therapists
KNOWLEDGE, SKILL, AND COMPETENCY REQUIREMENTS
Bachelor's degree in related field is preferred
Current certification through an accredited program of Massage Therapy
Current Massage Therapists license is valid in the state of Tennessee
CPR certification is required
Minimum one year of professional massage therapy experience required; experience in a healthcare setting is preferred
Participate in professional development activities and maintain professional affiliations
Excellent knowledge in physiology and working knowledge of human anatomy and orthopedics
Neat, professional appearance along with kind and helpful personality
Handle difficult situations in a calm and professional manner
Superior verbal and written communication skills
