Candidate Qualifications and Experience

Successful completion of HCBH pre-employment screening and background check.
Has the ability to communicate effectively orally and in writing.
Has a valid drivers license and eligible for HCBH insurance coverage(s).

The Physical Therapist Assistant (PTA) works under the supervision of a licensed Physical Therapist, Rehab Supervisor, and Rehab Director to deliver rehabilitative care that helps patients improve mobility, regain function, and adapt to physical limitations. The PTA implements individualized treatment plans, monitors patient progress, and communicates findings to the supervising therapist. This role involves providing therapeutic exercises, functional training, and use of modalities, while also ensuring accurate documentation and supporting program development and departmental goals. All care is delivered in accordance with state licensure requirements, APTA standards, and applicable safety regulations.
ESSENTIAL FUNCTIONS
Responsible for following the Physical Therapist's plan of care, performing interventions to meet the goals of the plan, progressing patients appropriately, and accurately utilizing CPT codes.
Sees all patients as scheduled and assists with other patient needs throughout the continuum if needed, demonstrating fiscal responsibility, accountability, time management and prioritization skills.
Completes daily treatment notes with pertinent information charges, and other pertinent information per CCH policy.
Works scheduled work hours and participates in weekend coverage to meet patient needs is flexible during times of high and low census and will provide coverage to other CCH service lines as needed.
Initiates review of patient plans, progress notes, and supervisory meeting with evaluating therapists.
Provides service excellence to all customers and positive interpersonal relations with all team members.
Maintains professional standards related to clinical practice, staffing and continuing education.
Maintains a clean and safe working environment that is conducive to high quality care
Complies with the hospital's Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.

Essential Duties and Responsibilities:
Under the direction of the licensed physical therapist, optimize the functional abilities and skills of patients.
Build a strong physical therapy program.
Enhance clinical expertise, professional and management skills through interaction with managers, therapists and other professional staff, self-study, and other continuing education activities.
Maintain a valid state license. Maintain awareness of issues related to the profession of physical therapy and the health care environment.
Understands various relevant payment models related to billing and treatment guidelines, as well as clinically appropriate means/modes of delivery. Provide quality care as well as reasonable goals and outcomes within the guidelines.
Be compliant with infection control procedures and environmental safety protocol within a facility.
Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports

Position Overview
Under the direction of the Clinical Director, the Clinical Therapist provides evidence-based treatment services designed to produce desired results. Candidate Qualifications and Experience
We're looking for passionate professionals who meet the following criteria:
Masters Degree in a Human-Services related field.
Licensed or licensable to practice therapy in the State of Wyoming and/or Idaho; OR state-recognized certification in the field of addiction.
Knowledge and understanding of competent treatment, including administering clinical evaluations, treatment planning, appropriate referrals, service coordination, documentation practices, and professional ethics.
High degree of organizational skills and attention to detail.
Ability to communicate effectively orally and in writing.
Proficiency in Microsoft Office (Word, PowerPoint, Excel, Outlook).
Must possess a valid drivers license and be eligible for HCBH insurance coverage(s).
Successful completion of HCBH pre-employment screening and background check.
