IOP COORDINATOR
Horizon Health is seeking an IOP coordinator to join our team at the IOP at Unity Point Health- Finley Hospital in Dubuque, IA. In addition to providing direct high quality psychotherapy services to high functioning patients in assessments, diagnoses, treatment planning, group treatment, individual treatment, family treatment, and discharge planning for the Intensive Outpatient Program, IOP Coordinator also plans and implements programs to meet the social and emotional needs of the patients. The Social Services Coordinator effectively participates as a member of the multidisciplinary team and the unit community relations team.
Responsibilities:
Complete psychosocial assessment, Plan of Action, and Plan of Action updates within established timeframe.
Develops and implements program plans for patient care during treatment and release from the program
Collaborate with psychiatrist directing the group to ensure quality continuation of care and schedule weekly medication management
Responsible for managing social services provided to patients and their families
Maintains compliance with state and federal confidentiality laws as well as upholding Horizon's confidentiality policies and procedures
Conducts individual, family, and group sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques
Submits timely and accurate CQI+ outcome data on a weekly basis
Attends Unit and Hospital meetings as directed
Participates as an active member of the Referral Development Team
Other duties as assigned
Supervisory Responsibility
The Social Services Coordinator may fulfill the role of supervisor for the program in the absence of the Program Director and/or during weekend shifts

Qualifications:
Minimum of one year of clinical massage therapy experience; two years' experience preferred

Ability to recognize client/family strengths and weaknesses as well as their support systems.

Position Information:
Are you a passionate MassageTherapist looking to grow your career in a dynamic, integrative care setting? Join us at the NWHSU Bloomington Clinic-Northwestern Health Sciences University's largest and most established clinic, with over 30 years of providing exceptional, patient-centered care. By conducting initial assessments and reviewing patient history, you will work with clients to define health goals and provide therapeutic massage care. This role also includes clinical assisting duties, including creating smooth patient workflow by assisting providers, rooming patients, maintaining OSHA cleaning standards, and educating patients per providers instructions. This is a unique opportunity to work with a diverse client base and collaborate alongside a multidisciplinary team of experienced providers.
This position is a part time, benefits eligible role, working 20-29 hours per week; potential for hours to grow as demand increases.
Work Location: Bloomington, MN in-person
MassageTherapist Duties:
* Manages client care with the approved policies and procedures of the NWHSU clinic system and within clinical scope of practice.
* Provide the highest quality care possible utilizing evidence informed practices and assist each patient to reach their goals while maintaining a strong provider/client relationship ensuring client satisfaction.
* Discipline appropriate initial assessment of client based on history and observations.
* Monitors client progress and ensures appropriate follow-up care.
* Refer clients to internal or external healthcare professions as necessary and indicated by clinical findings.
* Complete all necessary client documentation in a timely manner, according to clinic policy.
* Conduct informed consent with each new clients prior to treatment and on an annual basis.
* Participate in public speaking, community event activities, and/or publish articles in newspaper/media sources for purposes of increasing patient utilization of University clinics.
* Attend meetings and required clinic in services i.e.: coding and documentation; HIPAA training; CPR; blood-borne pathogen, etc.
* Maintain licensure.
What you need to have?
* High school diploma or GED plus completion of an accredited massage therapy program.
* 1-3 years experience working in clinical settings.
* Eligible to sit for Board Certification in Therapeutic Massage and Body Work exam, required.
* Certified in Craniosacral Therapy, Lymphatic Drainage, Shiatsu, and CPR/BLS, preferred.
