The Obstetric and Gynecology physician provides health assessment and direct care to obstetric and gynecology patients as part of the provider team providing services to Matthew Walker Comprehensive Health Center patients in our Nashville, Smyrna and Clarksville, TN site locations. Position duties include; but, not limited to: Provides primary care along established guidelines of MWCHC protocols. The physician develops a plan of care for each patient, including complete history, physical examination, diagnosis, appropriate treatment and /or referral, used all available resources in an appropriate and cost efficient manner. Confers with consulting physicians, midlevel providers, nurses, support staff, patients, and families concerning treatment and care of patients. Reporting to the Community Outreach Coordinator the Community Health Worker (CHW) will be responsible for responsible for helping individuals and their families to navigate and access community services, other resources, and adopt healthy behaviors. Though outreach and in the clinic, activities will promote, maintain, and improve the health of patients and their family. The CHW provides social support and advocates for individuals and community health needs, and provides services such as needs assessments, health education, outreach, assistance support and coordination to obtain needed resources, such as health care, transportation, food, housing, and employment. CHWs usually share ethnicity, language, socioeconomic status, and life experiences with the community members they serve. The community members served are Black or African American, Hispanic or Latinos and Immigrant and Refugee communities in North or South/Southeast Nashville. SUMMARY: The LPC or LCSW will have primary responsibility for coordinating all psychosocial community services under the general direction of the Behavioral Health Manager. Additionally, responsible for implementing community educational programs, assessing problems and determining appropriate types and methods of treatment, and implementing intervention methodologies for mental health and substance abuse; acts as Program Consultant to staff members with or without professional training; functions in such areas as counseling, protective services, and family services.
