How you make a difference
The Mental Health Professional provides clinical services to individuals under the supervision of the Mental Health Coordinator, Clinical Supervisor or Mental Health Director. They conduct assessments, develop treatment plans, and provide evidenced-based psychotherapeutic interventions. Additionally, they provide mental health consultation and training to facility staff.
Key Responsibilities
* Provide clinical services to persons served in compliance with policies and relevant professional standards, including individual and group psychotherapy, crisis intervention, and diagnostic evaluations.
* Complete relevant clinical documentation, including clinical service reports and mental health treatment records, in a timely and professional manner.
* Provide consultation services to institutional staff, including screening committees and psychiatric staff, regarding the mental status and management of persons served.
* Participate in staff meetings, in-service training programs, and specialized clinical services and/or program development activities.
* Maintain accountability for services provided through participation in the Quality Improvement program and timely and accurate recording of activities.
Additional Details
Applicants for this position must have one of the following active licenses: Licensed Professional Counselor Licensed Clinical Social Worker Non-independent licenses such as Licensed Professional Counselor-In Training (LPC-IT) or Advanced Practice Social Worker (APSW) will be considered with relevant experience. Provide client care as directed by the Director of Nursing or RN supervisor.
Assist with personal care (bathing, grooming, dressing, toileting).
Help with feeding and monitoring vital signs (temperature, pulse, respiration, blood pressure).
Prevent skin breakdown through repositioning and range-of-motion exercises.
Observe and report changes in client's physical, mental, or emotional condition.
Support medication self-administration (reminders only).
Perform light housekeeping, meal prep, bed-making, and maintain a clean environment.
Ensure infection control, client safety, and adherence to care plans.
Provide transport for appointments or activities and participate in client teaching as needed.
Respond to emergencies and use equipment safely.
Required Skills
CNA certification.
Current physical or TB/PPD test, if applicable.
Valid driver's license with insured, reliable transportation.
Ability to work independently, communicate effectively, and maintain confidentiality.
Empathy, teamwork, and respect for clients and staff.
Ability to travel up to 30 minutes for client care, training, or business needs.

Responsibilities:
* Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
* Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
* Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
* Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
