Responsibilities:
+ Responsible for assisting patient with and per plan of care, the following;
+ Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
+ Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
+ Safe transfer and ambulation techniques
+ Exercise according to the plan of care
+ Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
+ Patient's laundry
+ Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
+ Observation and teaching of home safety practices
+ Recognition of an emergency situation and knowledge of emergency management procedures
+ Must follow patients plan of care and complete all tasks assigned
+ Communicates in a constant, clear, and effective way with the client, family/caregiver and office

Requirements:
+ Meets all training requirements in accordance with state and federal laws when applicable

Salary: $13 - $16 / per hour The State Tested Nursing Assistant (STNA) is a member of the home care team who works under the supervision of a Registered Nurse or Therapist. The STNA performs various personal care services, as necessary, to meet the patient's needs. The STNA provides service in accordance with Maxim's philosophy, standards and policies.
Essential Duties and Responsibilities:
+ Observes and documents patient condition and reports any changes, to the assigned nurse supervisor.
+ Communicates in a constant, clear, and effective way with the client, family/caregiver and office

Job Responsibilities
Provide client-facing care as assigned

Train and educate field staff as needed to ensure services are provided in accordance with brand standards
Complete new caregiver introductions for home care cases and staffing clients
Participate in the intake of new clients and onboarding of new employees
Assist office staff with coordinating and executing employee and client appreciation activities throughout the year
Assist the office and staff with other duties as needed
Adhere to the policies, procedures, and protocols of the agency of employment
Adhere to HIPAA laws and ensure confidentiality and security of the client's medical information
Job Requirements
High school diploma or GED
Minimum one (1) year of documented home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred
Preferred: Successful completion of a nursing assistant or certified home health aide program
Certified CPR and negative TB skin test or chest x-ray
Must have a valid driver's license and auto insurance (proof of insurance is required)
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
Self-directing with the ability to work with little direct supervision
Ability to adhere to HIPAA and ensure confidentiality and security of the client's medical information
Ability to read, write, speak, and understand English and communicate effectively
Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B
Ability to travel within Northeast Ohio territory, up to 60 miles from Middleburg Heights in some cases
Must have flexible availability Monday through Friday and some weekends

Job Description
We are looking for a special individual with a great mix of compassion and desire to grow professionally and personally. Are you an experienced and caring HHA Home Health Aide with home health, private duty, or hospice experience?
We have a unique position with a mix of direct patient care, some office and training responsibilities. We are specifically looking for someone who has a lot of flexibility in their schedule and can work 40 hours or more per week, mostly Monday thru Friday but also some weekends.
Job ResponsibilitiesRapid Response & Readiness
40+ work hours must be completed during the following on-call schedule: Monday thru Friday 7:00 am to 11:00 pm
Every other Saturday and Sunday 7:00 am to 11:00 pm
On rare occasions, the field staff coordinator may be asked to complete an overnight shift if patient safety is at risk and all other coverage options have been exhausted
Be prepared to leave for a shift within 15 minutes during on-call hours (BrightStar will schedule shifts as far in advance as possible).
Maintain readiness by ensuring:
o Uniform and ID badge are worn
o Childcare is pre-arranged
o Cell phone is charged, and ringer is on
o Transportation is available
o Equipment and necessary supplies are always accessible
§ Support BrightStar office operations when not assigned to field shifts
Direct Client Care & Clinical Support
§ Deliver hands-on and hands-off care per the client's personalized plan, including:
o Personal hygiene (bathing, grooming, dressing, toileting)
o Mobility assistance (transfers, range of motion, walking)
o Nutrition and environment (feeding, meal prep, light housekeeping, linen changes)
o Vital signs, intake/output, medication support, and skin care
§ Provide transportation to appointments, shopping, and outdoor activities
§ Observe and document changes in client condition; report to Director of Nursing
§ Uphold infection control standards and ensure a safe and respectful care environment
§ Respond appropriately to emergencies and use equipment safely

Administrative Support
§ Conduct caregiver introductions and assist with new client intakes
§ Support onboarding of new employees and assess field staff competency

§ Participate in performance improvement and client teaching activities
Assist with scheduling, phone support, and general office operations Job Qualifications & Requirements
Education & Experience
· High school diploma or GED
· Preferred: Associate or bachelor's degree (or actively pursuing)
· Minimum of three (3) years of personal care experience, including at least one (1) year as an HHA
· Preferred: One (1) year experience in a home health agency or healthcare management
Compliance & Confidentiality
· Ability to handle confidential information and enter into confidentiality agreements
· Ensures security of client medical information and BrightStar office data
· Adheres to BrightStar Care policies, HIPAA regulations, and client rights
· Ability to pass drug screening, background check, and criminal record clearance upon hire and ongoing
Communication & Collaboration
· Excellent customer service and tactful communication with clients, staff, and community partners
