Salary: $35 - $40 / per hour FULL TIME AVAILABILITY REQUIRED 1 Year of Nursing Experience Required - Tracheostomy and Ventilator experience preferred Maxim Healthcare is hiring for a Registered Nurse (RN) to work on our Transition team, for full-time night shifts. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.
Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then phased out of the home.

Transition Team Pay Scale
+ RN $35-40
Responsibilities:
· Utilize the nursing process to assess, plan, implement, and evaluate patient care.
· On each assignment, provides skilled nursing care/services in accordance with prescribed orders
· Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
· Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Qualifications:
· Currently licensed as an LPN/LVN in the state in which the LPN/LVN will practice.
· Current TB or Chest X-Ray.
· Current BLS card.

Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery.
Assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.).
Administer medication, insulin, and IV/fluids; documenting thoroughly.
Inspect and care for wounds, changing dressings, and assisting with personal hygiene.
Assess patients for further injuries, infections, bedsores, etc., and deliver appropriate treatments.
Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.
To qualify for a Registered Nurse (RN) with us, you will need:
Education: Graduate of an accredited school of nursing.
Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position.
Current CPR/AED/BLS/First Aid certification.
Reliable transportation to/from care sites and/or work locations.

Job Responsibilities
Rapid Response & Readiness
40+ work hours (on-call) must be completed during the following on-call schedule:
Monday thru Friday 8:00 am to 8:00 pm
Every other Saturday and Sunday 8:00 am to 8:00 pm
On rare occasions, the nurse may be asked to complete an overnight shift if patient safety is at risk and all other coverage options have been exhausted
Be prepared to leave for a shift within 30 minutes during on-call hours (BrightStar will schedule shifts as far in advance as possible)
Maintain readiness by ensuring:
Uniform and ID badge are worn
Childcare is pre-arranged
Cell phone is charged, and ringer is on
Transportation is available
Equipment and necessary supplies are always accessible
Support BrightStar office operations when not assigned to field shifts
Direct Client Care & Case Management
Conduct initial assessments, re-assessments, and safety evaluations
Deliver skilled nursing per Ohio Nurse Practice Act (e.g., infusion therapies, wound care, injections, etc.)
Observe and report changes in client condition, document thoroughly
Follow infection control protocols and universal precautions
Respond appropriately in emergencies and use equipment safely
Administrative Support
Develop and maintain person-centered Plans of Care and obtain physician signatures
Coordinate care with families, providers, and internal staff
Assist with referral intake and follow-up communications
Assist Clinical Director with:
Weekly chart reviews for compliance (JCAHO, state, brand standards)
Incident reporting and resolution
Timely client discharges with complete summaries
Daily documentation review to support billing and reimbursement
Supervise and coach nursing/caregiving staff:
Conduct competency assessments and supervisory visits
Lead caregiver-client introductions and field education
Facilitate orientation, skills training, and in-service programs
Be available for urgent clinical issues or questions

Licensure & Education
Current, unrestricted Registered Nurse (RN) license in the State of Ohio
Degree in Nursing from a National League for Nursing-accredited program
Minimum of three (3) years clinical experience, including at least one (1) year of documented case management
Preferred: One (1) year experience in a home health agency and healthcare management
Preferred: One (1) year experience working with pediatric clients

RN - Home Health Transition Team

Salary: $35 - $40 / per hour FULL TIME AVAILABILITY REQUIRED 1 Year of Nursing Experience Required - Tracheostomy and Ventilator experience preferred Maxim Healthcare is hiring for a Registered Nurse (RN) to work on our Transition team, for full-time night shifts. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.
Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then phased out of the home.
