Salary: $35 - $40 / per hour FULL TIME AVAILABILITY REQUIRED 1 Year of Nursing Experience Required - Tracheostomy and Ventilator experience preferred Maxim Healthcare is hiring for a RegisteredNurse (RN) to work on our Transition team, for full-time night shifts. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.
Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcarenurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then phased out of the home.

Transition Team Pay Scale
+ RN $35-40
Responsibilities:
· Utilize the nursing process to assess, plan, implement, and evaluate patient care.
· On each assignment, provides skilled nursing care/services in accordance with prescribed orders
· Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
· Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Qualifications:
· Currently licensed as an LPN/LVN in the state in which the LPN/LVN will practice.
· Current TB or Chest X-Ray.
· Current BLS card.

Job Responsibilities
Rapid Response & Readiness
Flexible 24 work hours must be completed during the following hours: Monday thru Friday 8:00 am to 8:00 pm
Overtime available
Some weekend hours available
Be prepared to leave for a shift within 30 minutes during on-call hours (BrightStar will schedule shifts as far in advance as possible)
Maintain readiness by ensuring:
Uniform and ID badge are worn

Support BrightStar office operations when not assigned to field shifts
Direct Client Care & Case Management
Conduct initial assessments, re-assessments, and safety evaluations
Deliver skilled nursing per OhioNurse Practice Act (e.g., infusion therapies, wound care, injections, etc.)
Observe and report changes in client condition, document thoroughly
Follow infection control protocols and universal precautions
Respond appropriately in emergencies and use equipment safely
Administrative Support
Develop and maintain person-centered Plans of Care and obtain physician signatures
Coordinate care with families, providers, and internal staff
Assist with referral intake and follow-up communications
Assist Clinical Director with:
Weekly chart reviews for compliance (JCAHO, state, brand standards)
Incident reporting and resolution
Timely client discharges with complete summaries
Daily documentation review to support billing and reimbursement
Supervise and coach nursing/caregiving staff:
Conduct competency assessments and supervisory visits
Lead caregiver-client introductions and field education
Facilitate orientation, skills training, and in-service programs
Be available for urgent clinical issues or questions
Job Requirements
Licensure & Education
Current, unrestricted RegisteredNurse (RN) license in the State of Ohio
Degree in Nursing from a National League for Nursing-accredited program
Minimum of three (3) years clinical experience, including at least one (1) year of documented case management
Preferred: One (1) year experience in a home health agency and healthcare management
Preferred: One (1) year experience working with pediatric clients
Clinical & Regulatory Competencies
Strong nursing assessment skills and attention to detail
Ability to supervise, educate, and direct staff effectively
Thorough understanding of regulatory compliance at state and federal levels
Familiarity with Joint Commission policies and survey preparedness
Adherence to HIPAA standards and commitment to client confidentiality
Ability to manage complaints and complex situations using sound clinical judgment
Experience in client teaching and public speaking

Valid driver's license, reliable transportation, and current auto insurance
Willingness to travel daily throughout the assigned territory, including weekends, evenings, and holidays
Current CPR certification and negative TB test within the past 12 months
Up-to-date vaccinations: Influenza, COVID-19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, Hepatitis B
Ability to pass drug screening, background check, and criminal record clearance upon hire and any beyond
Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds

As a Home HealthRegisteredNurse, here's a big-picture view of what you'll do:
Provide home-based care to patients with illnesses, injuries and chronic diseases-often after discharge from a hospital, rehab or nursing facility
Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
Examine patients, do assessments, document progress and report changes to their physician
Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
Coach family members on the patient's plan of care, medication and home safety
Ensure goals are met and coordinate discharge from services
Typically has a case load of 30-35 patients
Assigned visits per day based on a point system which averages 5-7 visits per day
A few must-haves for Home HealthRegisteredNurses:
Graduate of an accredited nursing program and active RN license in Ohio
