The Center for Human Development, (CHD) is seeking a Case Manager to join our team at Grace House, a residential treatment program for mothers in early recovery. This large historical residential home is located in Northampton, MA. The program can house up to twelve mothers with children in a collective living environment, where they share space and resources in an effort to build a recovery community and encourage peaceful living. Your role as a Case Manager: The Case Manager provides support services to women with children and/or pregnant in a comprehensive six-to-nine -month residential treatment program. S/he assists referred individuals in need of substance abuse treatment by providing the education and support needed to establish a clean and sober way of independent living and assisting clients to access community resources as well as mitigate barriers to success. As a therapeutic setting, we strive to provide each family with a safe, nurturing environment where mothers and their children can experience success. The Clinician provides clinical services within their assigned group home working with youth ranging in age from 0 - 21 years old that are referred by the Department of Children and Families (DCF). The Clinician works within the agency's team model, establishing open communication and support within the program. They may coordinate and complete intakes and coordinate clinical treatment within the program.
