Responsibilities:
Job Summary: The Physical Therapist is responsible for planning and administering physical therapy treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients.
Completes patient evaluations based on information obtained through history, observation, subjective and objective measurements. Develops plan of care most appropriate to the patient's learning style and therapeutic needs, communicating goals with the patient and/or family members, considering psycho-social, cultural and spiritual needs of patients and/or families.
Provides therapeutic interventions for a wide variety of physical and/or functional limitations related to, but not limited to musculoskeletal, neurological, cardiovascular, and developmental impairments, documenting progress with measurable and functional goals.
Maintain patient records according to hospital policy, completing evaluations, re-evaluations, progress and treatment notes, charges, and discharge summaries within departmental time guidelines.
Assists in adjusting schedules to accommodate changes in caseload in the department. Supervises operation of specific treatment team. Meets with PTA on regular basis to discuss patient and set goals.
Contributes to the overall performance of the department through professional development strategies, including, but not limited to competency training, clinical instructor, in service coordination, serving as a mentor in area of clinical expertise, program leader, attendance of meetings, and performance improvement ideas.
Demonstrates continued professional growth through setting goals and participating in education offerings, independently achieve and maintain CEU's for SC LLR.
Communicates with physicians, other medical personnel and members of the interdisciplinary medical team regarding treatment provided, patient's progress or complications, recommendations, and clarification of orders.
Utilizes appropriate health and safety equipment to protect both patients and staff. Ensures an orderly, clean and safe work area.
Performs other duties as assigned.

Physical Therapist - Home Health - Dillon * $15,000 Sign On Available * $7,500 Student Loan Repayment for up to 7 years * $7,500 Location Relocation Assistance Responsibilities: * Responsible for providing physical therapy services in accordance with the physician's written orders. Interacts daily with patients, family members and agency personnel and on a regular basis with physicians.
* Directly accountable to the McLeod Home Health Administrator. Functions within agency and unit policies and procedures. Manages own time productively and independently and complies with attendance and reporting requirements. Works closely with other staff, students, and other external agencies and groups. Contributes to the overall performance of the department through professional development strategies, including, but not limited to: competency training, clinical preceptor, in service coordination, serving as a mentor in area of clinical expertise. For therapy services will function as clinical education coordinator for education of students and volunteers and assist with performance improvement.
* Demonstrates knowledge of state therapy laws, the Conditions of Participation of the Federal Health Insurance Program for the Aged and Disable (Title XVIII, Social Security Act, Section 1861), and the regulations relating to Conditions of Participation for Home Health Agencies (Part 405, Section 1201-1243), ACHC, JCAHO and all other pertinent regulatory issues, must be knowledgeable about conflict resolution techniques and customer relations. Must have understanding of all agency services and referral systems. Must possess knowledge of community resources available to client base.
* Demonstrates expertise in communication and interpersonal relationships. Must possess excellent writing skills. Must be able to use computer. Must have excellent organizational skills. highly adaptable and have the ability to exercise high degree of initiative and judgment in carrying out work assignments. Must have excellent critical thinking and problem-solving skills.
* Ability to function in an office setting which includes standing, walking, sitting, etc. must be able to use office equipment such as copier, calculators, etc. must have a very good manual dexterity in carrying out technical and physical aspects of clinical care and operating clinical equipment. Must have good hearing and eyesight. Moderate physical activity involved, with daily and frequent walking, standing, bending, lifting and carrying of objects weighing up to fifty pounds, sitting and driving. Full range of motion and strenuous movements necessary when performing patient care.
* Responsible for handling potential problems related to safety issues for staff and patients in all settings. High degree of maturity, self-motivation, creativity, initiative, and independent judgment required. Must have responsible, ethical, self-initiating professional behavior. Effective interpersonal skills, empathy, and ability to listen and plan with staff, patients/customer/physicians are essential characteristics. Must be able to efficiently communicate with a diverse population. Self-awareness and self-evaluation are important characteristics as are accuracy in collecting data, completion of records and follow-up.

Responsibilities: Job Summary: The Physical Therapist is responsible for planning and administering physical therapy treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients.
* Completes patient evaluations based on information obtained through history, observation, subjective and objective measurements. Develops plan of care most appropriate to the patient's learning style and therapeutic needs, communicating goals with the patient and/or family members, considering psycho-social, cultural and spiritual needs of patients and/or families.
* Provides therapeutic interventions for a wide variety of physical and/or functional limitations related to, but not limited to musculoskeletal, neurological, cardiovascular, and developmental impairments, documenting progress with measurable and functional goals.
* Maintain patient records according to hospital policy, completing evaluations, re-evaluations, progress and treatment notes, charges, and discharge summaries within departmental time guidelines.
* Assists in adjusting schedules to accommodate changes in caseload in the department. Supervises operation of specific treatment team. Meets with PTA on regular basis to discuss patient and set goals.
* Contributes to the overall performance of the department through professional development strategies, including, but not limited to competency training, clinical instructor, in service coordination, serving as a mentor in area of clinical expertise, program leader, attendance of meetings, and performance improvement ideas.
