The Care Coordinator is responsible for scheduling, organizing, and managing all aspects of member healthcare maintenance and treatment. Help patients complete paperwork, communicate with a healthcare team about patient treatment plan, and educate patients about resources and options for managing their health.
WHERE YOULL WORK
This position is hybrid. Work from home while servicing members in the Coachella region. You will have full control over your schedule when meeting members. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:30 am to 5:00 pm.
WHAT YOULL DO
Review and interview potential members to verify eligibility for enrollment in the Health Homes Program (HHP)
Complete referral process when necessary
Orient and educate members and their families by meeting with them to explain the role of the Care Coordinator
Initiate a care plan and provide educational information related to treatments, procedures, medications, and continuing care requirements in coordination with healthcare providers
Cross-functionally work with healthcare professionals, community and social support services, and other company employees
Document evidence of care in member record in a concise and timely manner using the EHR system
Routinely check in or schedule appointments with assigned members
Maintain documentation of all member encounters to complete established reporting requirements
Communicate with providers and care teams regarding member progress and care needs
Explain and translate information related to the members care to and on behalf of the member, when necessary
Identify and follow up on all referrals to assure continuity of care and member/family needs have been met
Reassess care plans to ensure effectiveness in achieving desired outcomes for members and their family
Address member questions and process member requests in a timely manner
Investigate and direct member inquiries or complaints to appropriate staff members, including follow-ups to ensure satisfactory resolution
Follow established policies to enroll and disenroll members
Maintain knowledge of community services and resources available to members including housing resources, financial/income assistance, transportation assistance, and food assistance
Follow established protocol around patients experiencing pain, safety concerns, and/or mental health concerns to create safety plans, if necessary
Recognize signs of child and elder abuse and report appropriately to Child/Adult Protective Services
Understand and abide by all departmental and companywide policies and procedures
Comply with all safety and injury prevention policies and regulations

POSITION OVERVIEW:
The Clinical Supervisor is responsible for all clinical aspects of treatment for the patients they oversee. This includes the assessment and analysis of the patient's skills and challenging behaviors, development of treatment plans, overseeing the implementation of treatment, collaboration with and training of their patient's caregivers, as well as ongoing coaching and training of behavioral technicians. ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Uses clinical judgment to promote optimal outcomes for each patient
• Develop and maintain treatment plans
• Ensure that all supervision hours are at 100% treatment adherence each month
• Evaluate patients to identify both skill deficits and strengths
• Analyze challenging behaviors to identify the function of the behavior
• Develop functionally relevant treatment plans to reduce challenging behaviors

WORK ENVIRONMENT:

Responsibilities:
Program Management: Oversee ABA programs in clinic-based, in-home, school, and Telehealth settings. Ensure the effective implementation of behavior intervention plans and adherence to ethical guidelines and regulatory standards.
Team Leadership: Provide leadership, guidance, and supervision to a team of BCBA clinicians. Conduct regular meetings, case conferences, and performance evaluations to support professional growth.
Quality Assurance: Establish and maintain quality assurance processes to monitor and evaluate program effectiveness, treatment fidelity, and adherence to best ABA practices. Implement strategies for continuous program improvement.
Program Development: Identify areas for program growth and development, working closely with the executive team to align with industry trends, client needs, and organizational goals.
Staff Training: Develop and deliver training programs to enhance staff skills and knowledge in ABA principles, techniques, and ethical guidelines. Foster a culture of continuous learning.
Collaboration: Collaborate with interdisciplinary team members to ensure coordinated care. Maintain effective communication channels with stakeholders at all levels.
Client Case Management: Oversee client caseloads, monitor progress, and guide complex cases. Review assessment reports.
Data Analysis: Analyze client data and program outcomes, prepare comprehensive reports to communicate results to stakeholders.
Compliance: Ensure compliance with legal and regulatory requirements. Maintain accurate client records.
Marketing: Collaborate with the marketing team to promote ABA programs and increase community awareness. Represent the organization at conferences and events.
Schedule
3 days per week: In-clinic work
2 days per week: Remote "virtual office" work
Required virtual office hours: Minimum 5 hours daily during peak service hours on remote days
Remaining hours flexible within standard business hours
Key Responsibilities
In-Clinic Days (3 days per week):
Conduct in-person assessments and develop behavior intervention plans
Provide direct therapy and supervision to clients
Train and supervise Behavior Technicians in-person
Collaborate with the clinical team and other therapists
Attend in-person team meetings and case reviews
Engage in parent/caregiver training sessions
Maintain and update clinical documentation
Remote "Virtual Office" Days (2 days per week):
Maintain a consistent virtual office presence for a minimum of 5 hours daily during peak service hours
Provide remote supervision and support to clinics via video conferencing
Conduct telehealth sessions for appropriate cases
Analyze data and update treatment plans
Complete and review clinical documentation
Participate in virtual team meetings and case reviews
Provide remote training and support to Behavior Technicians
Engage in professional development activities
Be available during set working hours for consultation and support
Requirements:
Minimum 6 months of BCBA experience or equivalent clinical experience as an LMFT
Experience in functional behavior analyses/assessments
Basic knowledge of autism spectrum disorder and developmental disabilities
Capability to implement ABA methodologies
Master's degree in applied behavior analysis, education, psychology, or related field
BCBA certification OR LMFT license

What you will do:
* Perform routine teller transactions working directly with clients. Accepts, verifies, and processes basic to more complex customer transactions according to established procedures and security guidelines. Transactions include deposits, withdrawals, check cashing, payments, transfers, and transactions received through the mail or the night drop. Works quickly to service customers waiting in line, but maintains close attention to detail and complies with all policies, procedures and regulatory guidelines.
* Responsible for maintaining and balancing a cash supply for transactions performed. Adheres to all cash handling policies and procedures and balances cash at the end of each shift.
* Provides exemplary customer service. Represents the Bank and its products and services with pride and enthusiasm. Identify customer needs, provide information on products and services. Who you are:
