The CNA should notify the case manager or clinical supervisor of any deviations from the Activity Plan.
The CNA should report changes in patient's condition, including pain control to case manager or clinical supervisor within one hour of assessment
The CNA should accurately complete appropriate clinical records related to assigned duties and observations of patients within 48 hours of the provision of care
The CNA should work effectively as a member of the interdisciplinary team and contributes to plan of treatment based on knowledge and experience.
Requirements
CERTIFIED NURSING ASSISTANT QUALIFICATIONS:
Satisfactory completion of training program or competency test for Home Health aide which meets the requirements of the State of Wisconsin Department of Health as well as Medicare (CNA).
Home health and/or hospice experience preferred.
Current Driver's license.
Name is listed on the Wisconsin registry as current and eligible to provide home health and hospice care.
Excellent communication skills.
Evidence of attendance of hospice training class or willingness to attend.
Believes in and has a working knowledge of the hospice philosophy of care.
Basic Computer skills.

Responsibilities
The medicalassistant performs assigned tasks of direct and indirect care in the ambulatory setting to meet the care needs of patients under the direct supervision of a physician (medical acts) or assigned supervisor. * Assist with patient care in all perioperative areas of the procedural area under the direct supervision of a registered nurse or Physician
* Handle equipment and supplies that is both clean/sterile and dirty/contaminated
* Activities focus on maintaining a safe, hazard free, clean environment that facilitates patient focused care.
Qualifications
Arizona: High school diploma or GED equivalent. Graduate from a MedicalAssistant diploma or associate degree program. Active MedicalAssistant certification (CMA), or registration (RMA) or Certified Clinical MedicalAssistant (CCMA) is required. Maintains Basic Life Support (BLS) competency.
Florida: High school diploma or GED equivalent. Graduate from a MedicalAssistant diploma or associate degree program or hold an active MedicalAssistant certification (CMA), or registration (RMA) or Certified Clinical MedicalAssistant (CCMA) certification. Maintains Basic Life Support (BLS) competency. Active MedicalAssistant certification (CMA), or registration (RMA) or Certified Clinical MedicalAssistant (CCMA) is preferred.
Midwest: High school diploma or GED equivalent. Graduate from a MedicalAssistant diploma or associate degree program or hold an active MedicalAssistant certification (CMA), or registration (RMA) or Certified Clinical MedicalAssistant (CCMA) certification.
