(Currently at 3 weeks out) Prints Enhanced Encounter Forms Identifies Medicare Wellness patients Optum forms as directed by Quality - LPN prints now Prepares patients for examination and treatment. Takes patient histories and vital signs Obtains vital signs (height, weight, BP, pulse, oxygen level) Reviews and updates medical hx, surgery hx, social hx, family hx, and advanced directives Completes medication reconciliation (verifies medication, dosage and instructions) Verifies patient pharmacy Completes depression and fall screenings at appropriate times Prepares exam and treatment rooms with necessary instruments. Gives injections and applies dressings as required. Obtains and runs testing as ordered (urine dip, glucose, EKG, PT/INR, urine drug screen, blood work, urine pregnancy, rapid strep, rapid influenza) Prepares and maintains supplies and equipment for treatments. Performs nurse visits - including education of physician's orders to patient and proper charting after visit. Assists providers in preparing for physical exams. Assist providers with procedures as needed (Pap, minor procedures) Assists with scheduling of tests and treatments and managing referrals in referral management Triage calls as needed Take off voice mail message at regular intervals Provides general information on departmental policies and procedures. Identifies clinical and office supplies and equipment needed and notifies lead and/or practice manager. Maintains patient files, records, and other information. Obtains information from paper charts or Allscripts (as needed) Requests records from outside offices or provides information to PSR Compiles and condenses technical and statistical data for reports and records.
