Responsibilities
Records details of chief complaint, history of present illness, review of systems, past medical history, social history, family medical history, disease risk factors, medications and allergies
Records details of physical exam, including pertinent positives and negatives, noted abnormalities and patient general condition.
Records orders as desired by medical provider and provides consistent updates to the provider for completion of those orders.
Assists in disposition of patient, whether discharged or admitted, including discharge instructions, admission orders, follow-up instructions, and documentation of prescription medications.
Ensures compliance with EMR-required measured data, such as E/M coding requirements, Meaningful Use criteria and documentation of Medical Decision Making.

Ensure medical reports for patients (i.e. initial, re-exam, and final) are completed and submitted to be reviewed and signed by the Chiropractors and Physical Therapists who completed that evaluation.
