A CNA Certified Nursing Assistant is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times.
CNA Certified Nursing Assistant QUALIFICATIONS
• Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant.
• A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled
in a nursing assistant certification class.
• Must possess a sincere desire to serve the ill, aged and infirm.
• Ability to read, write and speak the English language.
• Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general
public.

CNA Certified Nursing Assistant GENERAL DUTIES AND RESPONSIBILITIES:
GENERAL
• Perform all duties as assigned and in accordance with facility's established policies and procedures, nursing care procedures and safety rules and regulations.
• Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care.
• Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately.
• Make beds, clean bedside and overbed tables.
• Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water
and within the resident's reach. Pass water pitcher as per established policy and procedure.
• Dress residents neatly and in their own clothing. Assist residents in dressing as necessary.
• Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting
in feeding or cutting food, removal of trays, supervision in dining room, etc.).
• Serve nourishment in accordance with established facility procedures.
• Feed residents who cannot feed themselves.
• Assure that resident's food is accessible and self-help devices are available as needed.
• Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities.
• Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary.
• Do not permit residents to smoke in bed.
• Assure that the nurse's call system is attached to the bed and within easy reach at all times when
residents are bedfast.
• Answer residents' call lights promptly.
• Check for food in residents' room. Ensure that food is kept in a sealed container.
• Report all complaints to the Charge Nurse.
• Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.)
• Report any medication found in the resident's room. Take to the Charge Nurse immediately.
• Report all hazardous conditions and faulty equipment to the Charge Nurse immediately.
• Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately.
• Attend in-service educational programs, on the job training programs and meetings as directed.
• Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
CLINICAL
• Wash hands before and after caring for each resident.
• Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on
enteral feeding.
• Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and
soaked with Efferdent after dinner.
• Record resident's food and nourishment intake as directed.
• Perform after meal care as required - cleaning resident's hands, face, clothing, dentures, etc.
• Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such
equipment after each use (document Intake and Output as directed).
• Check and document bowel movements and character of stools every shift.
• Bathe residents as assigned and in accordance with established facility procedures (encourage showers
and other self-help measures/activities).
• Comb and brush resident's hair daily. Shampoo hair on shower days.
• Clip and trim fingernails.
• Shave male residents daily or as needed.
• Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in
nose and ears as needed.
• Observe and report the presence of reddened areas on bony prominences.
• Keep drainage bag off the floor when resident is in bed or in wheelchair.
• Apply Vaseline or A & D ointment to dry skin in accordance with established policies.
• Move and turn bedfast residents at least every two hours and record such information on CNA notes.
• Observe disoriented and comatose residents and notify Charge Nurse immediately.
• Apply postural support properly. Check with Charge Nurse for type of restraint to be used.
• Apply hand roll or splint to residents with contractures as outlined within the facility's established
policies and procedures.
• Follow infection control procedures in accordance with established facility policy when isolation
techniques are necessary.
• Chart required information every shift.
• Complete Intake and Output documentation on all residents with orders before the end of each shift.
• Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
CONSUMER SERVICE
• Presents professional image to consumers through dress, behavior and speech.
• Adheres to Company Standards for resolving consumer concerns.
• Ensures that all consumer/resident rights are protected.

The position requires that the employee be available forty hours per week, and that also the employee be flexible in his/her schedule to provide efficient service for the group. The employee however, will be expected to work no more than five days per week.
Many of the responsibilities require that you have a working knowledge of computers and the ability to learn the proper use of the programs utilized in this office, or those that which may be necessary to meet the needs of the practice.

BASIC FUNCTION:
Under the supervision of the Back Office Supervisor, the Clinic MedicalAssistant shall be responsible for assisting their assigned provider in clinic with all aspects of the patients' visit.
RESPONSIBILITIES:
Work is primarily performed in the Clinic located in Arrowhead Orthopaedics' offices. The duties of the Clinic MedicalAssistant are of high volume, and the quality of duties performed is needed to be at a high level. The employee will come into contact on a daily basis with confidential patient files. The Clinic MedicalAssistant must be able to handle this information with the highest degree of privacy, discretion, and professionalism. The Clinic MedicalAssistant is required to be available to the physician and patients while in clinic, and is only authorized to leave an ongoing clinic with the direct permission by the Back Office Supervisor. The Clinic MedicalAssistant is responsible for accomplishing the duties set forth below:
General Activities (There may be other duties required of this position not listed below):
Ensure that authorization is complete for the patient visit.
Ensure that all necessary diagnostic studies and reports are available for the provider to review.
Ensure proper documentation in our EMR system for patients' visits. This includes, but is not limited to, using the appropriate template and completing the history portion including medication, past medical and surgical history, weighing all new patients, and taking vitals.
Document in EMR any changes in patients' conditions since last visit.
Bill all charges for services rendered and make sure that there is documentation in EMR justifying the charges.
Create proper documentation in EMR as needed for any diagnostic testing or therapy or surgical procedures ordered by the provider, and forward the appropriate authorization.
Act as the liaison between the patient and the provider.
Return all patient phone calls the same day and document in each patient's chart any instructions or concerns.
Work with the provider's Patient Services Representative and the Back Office Supervisor to ensure that the provider is scheduled appropriately.
Communicate with front desk staff any delay in the provider's schedule for the day.
Keep exam rooms stocked with medical supplies and linen.
Clean and empty rooms of dirty linen and red bag waste daily.
Clean dirty instruments and prepare them for sterilization on assigned day each week.
Perform other responsibilities as may be called on by the Back Office Supervisor.
RELATIONSHIPS:
The Clinic MedicalAssistant should observe and conduct the following relationships:
The employee shall be responsible directly to the Back Office Supervisor. The employee shall report to the Back Office Supervisor and his/her assignees any progress, reports, requests, concerns, problems, and/or expectations in relation to the responsibilities of the position. Such communication may be verbal or written as deemed necessary by the employee or as directed by the Back Office Supervisor or his/her assignee.
The employee shall interact with other office and clinical staff in the Practice. Such interactions should be collegial, professional, and contributing to the safety and pleasantness of the work environment at Arrowhead Orthopaedics.
The employee shall interact with patients, medical groups, adjustors, nurse case managers, hospitals, and Primary Care Physicians and their offices. Such communication should always reflect the total commitment of the office to quality care and customer satisfaction. All communication should be professional and any unresolved conflicts are to be reported to the Back Office Supervisor.

This role also interfaces with the quality/care management team to improve best practices. Medical Scribes are trained to be experts in structured clinical assessments, accurate and specific documentation and team based, patient-centered care.

Maintains records and assists with clinical paperwork (i.e., refills, orders, forms, referrals, etc).
Helps clean and maintain workstation and exam rooms.
Places orders and completes inbox items at the direction of the healthcare provider.
Work cooperatively with others, including appropriate communication with patients, providers, support staff, and administration.
Review and prepare patient charts prior to visit with Provider including review and preparation of the patient's chronic medical conditions and care quality measures.
Research existing chart documentation, legacy EHR, and outside medical records to discover and validate Risk Adjusted Diagnosis (HCC Codes) and build documentation in the patient's problem list and current encounter.
Observe and record Patient encounters/examinations by accompanying the Provider into the exam room or via virtual connection.
Transcribe Patient visits with accuracy and detailed information.
Transcribe the Patient history, physical exam findings, consultations, labs, x-rays, and other evaluations, as stated by the Provider.
Document any procedures, medications, injections, and tests that are either ordered or completed by the Provider.
Prepare care plans and patient facing instructions as directed by the Provider, communicate/collaborate with the care-team to assure that all elements of the care plan are addressed and completed.
Communicate with the care team including MedicalAssistants, Referrals Coordinator, Case Management, and scheduling team on any follow up, procedures or orders that may be needed.
Maintain up-to-date information for the Provider(s) during every visit, if necessary.
Prepare for and provide patient education documents, as needed. These may be from the EHR library or could be company specific.
Other duties as assigned.
Required education and experience: The requirements listed below are representative of the knowledge, skills, and/or ability required.
Minimum Education required:
High school diploma or equivalent (GED).
Bachelor's degree preferred but not required.
Minimum Experience Required:
Successful completion of a scribe training or medicalassisting program preferred.
Minimum Knowledge and Skills Required:
Bilingual Spanish is preferred.
Knowledge of medical terminology.
Ability to type 65+ words per minute.
Ability to communicate effectively and congenially with patients and staff members in person and over the phone.
Ability to exercise tact, initiative, and good judgement when interacting with patients and staff.
Ability to record observations and notes completely, accurately, literately, and concisely.
Ability to learn and code for Hierarchical Condition Categories.
Computer skills necessary to navigate the company's EMR system.
Ability to accept supervision and feedback.
Understand HCCs and Medicare.
