About the Role
As a Belmont Village Caregiver, you will provide hands-on personal assistance to a group of seniors living within our large Assisted Living and Memory Care community. The Caregiver role is ideal for someone who is dependable, detail oriented, patient, caring and enjoys a high level of personal engagement with older adults.
Your Typical Daily Responsibilities
Provide personal assistance with Activities of Daily Living to an assigned group of senior residents living in our Assisted Living and Memory Care community
Encourage socialization and inspire participation in community activities while ensuring resident safety and well-being
Document care and services provided daily by following customized service plans and communicating to supervisor any observed changes in care needs
Ensure care, safety and cleanliness standards are met according to regulatory compliance
Provide hands-on assistance with toileting, hygiene and incontinence care as needed
Requirements
Must be at least 18 years of age
High School diploma or equivalent
Must be able to communicate clearly in verbal and written English
Experience as a primary caregiver or relevant training as a care provider
Ability to work the defined schedule for this position which may include weekends and holidays

QUALIFICATIONS

Inventories and stores the new customer's personal possessions.
Assists customers with packing personal possessions when they are transferred to a new room or when being discharged.
Participates in and receives the nursing report upon reporting for duty.
Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools.
Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement.
Answers call lights promptly, answer telephone calls as needed and run errands as requested.
Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed.
Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse.
Observes customer's physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated.
Performs personal services for customer's (i.e., reading, writing letters, etc.) as time allows.
Encourages customers to help themselves and promotes self-care in aiding rehabilitation.
Assists with care of dying and expired customers.
Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident's eating habits to the Staff Nurse.
Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated.
Serves between meals and bedtime snacks.
Wears transfer belt at all times and utilizes as needed.
Keeps the nurses' call system within easy reach of the resident.
Reports all hazardous equipment and conditions to the Staff Nurse immediately.
Participates in the development, maintenance, and implementation of the facility's Quality Assurance program for the Nursing Department.

HOURS: PRN. Shifts will be Monday-Friday 12-hour shifts 7PM-7AM
TERRITORY: Crystal Lake, Lake in the Hills, Algonquin and surrounding area.
Qualifications:
Certified by the state as a nurse aide
List on the state aide registry with a clear record able to deliver care to patients
18 years of age unless state law dictates otherwise
Valid driver's license with an automobile that is insured in accordance with state requirements

What You'll Do:
Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens
Assisting in dressing and undressing patients
Planning and preparing nutritious meals, including shopping
Assisting in feeding the patient
Providing proper care and observation of patient's skin to prevent breakdown of tissue
Reporting on patient's condition and significant changes to the Case Manager.

Allure of Zion - SLF is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide routine daily care and services to residents in accordance with individualized assessments and care plans.
The ideal candidate is kind, patient-centered, and dedicated to resident dignity and comfort. We are looking for the next member of our Allure family - someone who truly loves working with residents.
Responsibilities
Assist residents with activities of daily living (ADLs) according to care plans
Measure and record vital signs, including temperature, pulse, respirations, blood pressure, and weight
Record food and fluid intake and report changes in eating habits
Assist with lifting, turning, positioning, transferring, and transporting residents
Provide care to residents with mental health conditions, psychosocial needs, or history of trauma
Assist non-diabetic residents with nail care (hands and feet) as permitted
Routinely check residents to ensure personal care needs are met per their wishes
Observe and report skin changes, pressure areas, or breakdowns
Collect specimens as instructed (urine, stool, sputum, etc.)
Assist nursing staff with admissions, transfers, and discharges
? Qualifications
Current Certified Nursing Assistant (CNA) certification (Illinois)
CPR certification required
Must be at least 18 years of age
Ability to read, write, speak, and understand English
Ability to work independently with integrity and professionalism
Ability to work effectively with residents, staff, and support agencies
? Physical Requirements
Ability to move intermittently throughout the workday
Ability to lift, push, pull, or carry up to 25 pounds and transport items up to 50 feet

About the Role
As a Belmont Village Caregiver, you will provide hands-on personal assistance to a group of seniors living within our large Assisted Living and Memory Care community. The Caregiver role is ideal for someone who is dependable, detail oriented, patient, caring and enjoys a high level of personal engagement with older adults.
Your Typical Daily Responsibilities
Provide personal assistance with Activities of Daily Living to an assigned group of senior residents living in our Assisted Living and Memory Care community
Encourage socialization and inspire participation in community activities while ensuring resident safety and well-being
Document care and services provided daily by following customized service plans and communicating to supervisor any observed changes in care needs
Ensure care, safety and cleanliness standards are met according to regulatory compliance
Provide hands-on assistance with toileting, hygiene and incontinence care as needed
Requirements
Must be at least 18 years of age
High School diploma or equivalent
Must be able to communicate clearly in verbal and written English
Experience as a primary caregiver or relevant training as a care provider
Ability to work the defined schedule for this position which may include weekends and holidays
Preferred Skills and Characteristics
You have at least one year of direct care experience in an Assisted Living, Memory Care or Skilled Nursing community
You follow detailed instruction while working as part of a team to achieve a shared goal
You are self-directed with strong organizational and time management skills
You are observant, dependable, adaptable and respectful
