The Kaizen will provide care to the elders living in a household. The CMA is required to pass medications that are within their scope of practice and will be directed by the nurse on duty. A Kaizen's primary role is to protect, sustain, and nurture the elders by providing assistance with activities of daily living, and meeting other needs as required. The core goal of the work of the Kaizen will be to provide assistance, which promotes a high quality of life and a positive elderhood. The responsibilities of the Kaizen include care of the elder, life enhancement programming and caring for the elders' environment, including cooking, laundry, and housekeeping. The Kaizen is a member of a team and is responsible for coordinating aspects of the team's work. Key Responsibilities/Duties Pass medications to elders within scope of med aide licensure. Report changes in the elder's condition to the clinical support team per neighborhood change in condition protocols. Assist elder with personal care functions, including dental and mouth care, bathing, dressing and grooming, bowel and bladder care and other personal and hygiene care. Meet with the clinical support team as scheduled or needed, to discuss elders needs and progress in identifying and correcting problems attend and contribute to the elders' scheduled care plan meetings. Report accidents and incidents when they occur. Assist with and/or perform restorative and rehabilitative procedures as outlined by the service agreement. Observe standard precautions per Brewster Infection Control policy(ies). Measure and record vital signs, such as temperature, pulse, respirations, weight, height, anatomical dimension/circumference, etc. Plan and implement life enhancement programming for each individual elder and document appropriately in clinical record. Record care data on electronic health record (EHR) for each elder at the time of service. You will provide onsite and administrative support to our APPs in the daily management of their practices, helping them to maximize their impact on conducting initiating patient visits for chronic care management services. Implements identified plan through coordination of care with interdisciplinary care team to employ strategies to promote health and wellness. Promote infection prevention through use of standard precautions, proper procedure in dressing changes, wound care, hand hygiene and cleanliness of the patient rooms/department Provide assistance with activities of daily living (ADL'S), accurate measurement and recording of weight, height, vital signs and report any changes to assigned nurse. Collects laboratory specimens through Point of Care testing, urine collection and or serum lab draws and complete procedures such as EKG, bladder scan, application of telemetry monitor as directed by nursing, removal of peripheral IVs and other duties assigned.
