Medical Records Coordinator POSITION SUMMARY: The Medical Records Coordinator is responsible for managing and maintaining accurate, secure, and compliant participant health records within electronic health systems. This role supports the interdisciplinary team by ensuring timely documentation, data integrity, and efficient record retrieval. Additionally, the specialist assists with audits, reporting, and system optimization to enhance care coordination and regulatory compliance. Strong attention to detail, technical proficiency, and a commitment to data security are essential. ESSENTIAL DUTIES AND RESPONSIBILITIES: Prepare and maintain participant charts, including filing, purging, and managing active and closed records. Administer and maintain the electronic medical record (EMR) system and paper records as required. Request and manage medical records related to institutionalizations (e.g., ED visits, hospitalizations). Provide orientation, troubleshooting, and updates for the EMR system; develop long-term strategies for electronic records management. Ensure compliance with HIPAA and government regulations for records storage, security, and maintenance. Develop and implement standards and guidelines for electronic records storage, retrieval, and preservation. Coordinate with the Compliance Officer to release information by HIPAA and Protected Health Information policies. Prepare EMR records for surveys, external agency reviews, and program needs. Send medical information as needed for participant intake and Interdisciplinary Team requests. Maintain confidentiality, privacy, and a safe working environment, adhering to OSHA and company policies. Support Quality Improvement initiatives and participate in continuing education and training. Perform additional duties as assigned. EXPERIENCE AND EDUCATION: Minimum of an associate's degree. Bachelor's degree in library/information sciences preferred One year of experience in Medical Records or other clerical setting in long term care environment preferred. A valid driver's license with a good driving record and an acceptable certificate of auto insurance. 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role). PRE-EMPLOYMENT REQUIREMENTS: Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact. Mission of Department: The Ohio County Hospital Nursing Department exists to provide quality nursing services to the patients/customers of Ohio County Hospital Corp., holistically and addressing physical, educational, mental, emotional, cultural, social and spiritual needs, by utilizing the nursing process. Purpose of Job: Performs designated tasks for patients in all age groups in clinic setting. May perform basic monitoring to include temperature, pulse, respiration, B/P. Tasks are performed in accordance to hospital and nursing policies and procedures as well as legal compliance policies. Must have good communication skills and support team work in the delivery of patient care as well as demonstrate customer relations consistent with the hospital's purpose, vision, and values. Experience/Education: Certification required. Keyboarding and basic computer skills preferred. Ability to follow written and/or verbal instructions effectively. Emotional stability. Knowledgeable of health care field, medical office protocols/procedures, medical terminology. OCH prefers that Non-Certified Clinical Assistants (those holding a SRNA certification or nothing) obtain a CCMA certification within 6 months of hire if they hold 1 year of clinical experience or within 1 year of employment with no clinical experience. Working Environment: Usual working space is well ventilated area and exam rooms. Requires standing, walking, turning, bending, stooping, and lifting patients. Direct patient care which requires both fine and gross motor skills. Also subject to exposure to bloodborne pathogens at all times in patient care. May have occasional exposure to radiation during radiology procedures and to dangerous chemicals (cleaning and disinfecting agents, rarely chemotherapeutic agents). Environment may be stressful at times, therefore ability to work under stressful conditions is necessary. Performs all procedures in accordance with established policies and procedures of OCH and adheres to legal compliance policies and other regulatory issues Performance Criteria: Demonstrates age appropriate skills in completing assigned patient activities. Transports patients in a safe and timely fashion. Collects specimens as assigned using correct policy and procedure for collection, labeling, and transporting specimens to the lab. Observed and record vital signs accurately and in a timely manner as assigned. Obtains weight and height on patients as assigned and/or according to policy. Cleans and prepares exam rooms following each patient and all medical histories. Accurately and promptly reports changes in patients' condition to the provider. Understands and utilizes EMR. Demonstrates competency in performing or assisting in assigned procedures within scope. Accurately obtains patient history/information. May perform simple lab and screening tests with competency. Provides for patient privacy during any treatment. Properly disinfects equipment as per policy. Completes medication reconciliations. May be responsible for scheduling of tests or referrals and communicating to patient. Maintains safe and therapeutic environment for patient care Takes necessary safety precautions. Reports any faulty equipment, infection or safety hazards to the appropriate personnel. Uses equipment and supplies in safe, efficient manner. Charge capture methods utilized according to established processes. Occurrence reports completed as required by policy; may request assistance in completion. Timeliness of completion is to be as soon as possible after the occurrence, but no later than by the end of the shift being worked. Knows and responds appropriately to all emergency codes. Reports to assigned area during emergency event and carries out assigned functions in timely manner. Knows and utilizes proper body mechanics. Reports any Environment of Care hazards/risks to the appropriate personnel. Assists other personnel in reducing clutter in the work environment - carts, equipment in the hallway. Applies Safety Guidelines as appropriate for specific patient (age appropriate - pediatric, geriatric, orthopedic, surgical, etc.) Makes appropriate requests for supplies or equipment needs. Accountable for ensuring patient safety during the course of performing duties. Upholds Standards of Nursing Practice for all age groups and Code of Ethics Code of Ethics and Standards of Nursing Practice are consistently met. Performs all assigned procedures according to the established policies and procedures of OCH. Recognizes and supports the Patients Rights. Maintains confidentiality in all hospital matters. Adheres to HIPAA policies. Adheres to Bloodborne Pathogen policies. Knows where to locate personal protective equipment and utilizes such appropriately. Adheres to infection control/prevention policies, including the exposure control plan and employee health protocols. Follows and upholds hospital personnel policies. Committed to continuous performance improvement/quality improvement and actively participates in and supports the organization's improvement efforts. Adheres to legal compliance policies. Maintains and continues professional self-development and exhibits positive human relations. Attends minimum of 50% of educational offerings that pertain to assigned areas and applicable. These educational offerings may include at least one hour on fire, safety, and mass disaster, and at least 2 hours on infection control/prevention. Maintain BLS provider status or equivalent knowledge. Maintain clinical skills as evidenced through competency testing and on-going performance as observed by supervisory staff. Accepts supervision. Seeks guidance/assistance as needed and accepts supervisory feedback non-defensively. Attends a minimum of 80% of staff meetings per year. Demonstrates a cooperative attitude when interacting with others. Tactful - maintains composure and is diplomatic in all situations. Treats all hospital associates, patients, family members, and visitors with courtesy, compassion and respect. Exhibits caring attitude toward coworkers. Willingly helps team members. Perfect attendance - exceeds standard Absent 1-5 occurrences - standard Absent more than 5 occurrences - does not meet standard, must improve 17. Cooperates with staffing needs; assists with covering open shifts in various clinics needed. Supports organizational accountability and demonstrates such through work. Qualifications High school diploma or GED. BLS provider status or equivalent knowledge. If does not have upon hire, will obtain within 6 months of hire. Certified Nurse Aide or Registered Medical Assistant.
