The Medical Records File Room Clerk position is a service-oriented position requiring a detail-oriented person with a professional and positive attitude. Their primary responsibilities include managing and processing various patient-related activities. Such activities include but are not limited to electronic documents that arrive at the fax lines, processing scheduled online exams, radiology films, CDs, and reports. The Clerk will also be responsible for accepting incoming phone calls on various phone lines. The Clerk will work in a corroborative, team environment by establishing strong working relationships with internal SMIL departments and external customers. The Clerk will consistently demonstrate exemplary performance in-line with SMIL's Performance Standards in Attendance, Quality, Productivity, Customer Service, Teamwork, Job Knowledge, Communication, Problem Solving, Initiative & Energy as well as adherence to Workplace Guidelines.
Essential Duties and Responsibilities
Demonstrates a commitment to “World Class Customer Service” and promotes a positive work environment.
Adheres to the SMIL Attaching Protocol by responding to Inbound Faxed Documents (orders, clinical notes, authorizations, etc.) via fax line ************, ************, and ************ as well as any other documents put into the Inbox of RIS; utilizing RIS Document Worklist.
Use appropriate patient search criteria when attempting to locate patient records to attach documents.
Attach and split multiple-page and multiple-patient document faxes.
Process all orders received electronically via Synapse RIS.
Adheres to STAT Request Protocol
Provides the initial contact with patients based on the orders received from referring physicians.
Ensures that orders are contacted within 24-48 hours and follow up as needed.
Completes data entry for all online scheduling completed through SMIL's Appointment Avenue tool.
Minimal scheduling and order entering within both Fuji Synapse RIS and the Appointment Avenue online Scheduling tool.
Responsible for setting up transportation services for patients' future appointments, as needed.
Identifies and reports to the Department Supervisor and/or Contact Center Coordinators issues regarding Document Management and individual status activity.
Process Outside Read Imaging
Process Outside Film from other Imaging Companies
Answering large volume of phone calls to the CFR for record requests from patients, referring physicians and internal staff.
Prepare and process requests for medical records.
Review medical records for completeness, assemble records into standard order and file records in designated areas according to request.
Assemble, sign out, and deliver medical records requested by patients, referring physicians and other internal departments.
Operate computer to enter and retrieve data.
Assist other workers with all Medical Record tasks.
Scan materials or copy records to maintain patient files.
Select and package files for movement, transfer, and permanent storage.
Transfer outside films/cd's and reports accurately into SMIL's PACS/RIS systems.

Document patient vitals, assessment, encounter type, and provider into EMR.
Communicate with physician and/or nurse practitioner, changes in patient condition, abnormal vitals, lab results, and abnormal patient response during testing/exam.
Prepare minor procedure trays and assist with procedures (i.e., hysteroscopy, endometrial and/or punch biopsy, cyst aspiration, Pap smears, GC/chlamydia testing, HSG testing, etc.).
Prep exam room respective to patient appointment and testing.
Educate patient on procedure(s) and provide answers prior to procedure.
Assist physician and/or nurse practitioner with patient examinations.
Maintain patient workflow to meet physician/nurse practitioner scheduling requirements.
Order and schedule lab tests as directed; review results and annotate appropriate disposition.
Order prescriptions/refills for patients prior to procedure; notify patient upon completion.

Order and maintain back-office supplies and reconcile monthly receipts/packing slips for submission to the business office.
Scan signed consents, office appointment forms, laboratory, and pathology reports into EMR.
Monitor medicine cabinet/refrigerator and order medications, as needed.
Perform urine pregnancy test, document results in EMR, and communicate with physician, nurse practitioner, or rad technician prior to exam/procedure.
Autoclave all used instruments, document size and type of instrument on package, change sterile/cleaning solution, and document in the soak-log.
Clean stationary room equipment, underside of exam tables, and provide clean/linens/supplies for all patient rooms' weekly minimum, or as needed.
Maintain HIPAA and Confidentiality for all patient and clinic information, HIV confidentiality.
Understand and assess potential domestic violence and child abuse issues and report concerns to Medical Director or Practice Administrator.

What You Bring:

Principal Duties and Responsibilities
1. Demonstrate knowledge of and application of immunotherapy protocols.
2. Demonstrate quality patient assessments relative to wellness for administration of immunotherapy, including breath sound assessments. Transition patients to provider care who present with acute illness.
3. Review immunotherapy consents with patients and answer patient questions concerning immunotherapy.
4. Perform all safety checks per protocol for the administration of immunotherapy and other injectable medications such as Xolair.
5. Administer accurate dosing and appropriate injection technique, adhering to OSHA guidelines.
6. Execute accurate and safe patient care including emergency care related to a reaction to immunotherapy, assisting the provider as needed.
7. Monitor patients during the immunotherapy 30-minute wait period and assess at the completion of the wait period or sooner as needed.
8. Perform serum titrations and special testing appropriate in the immunotherapy department.
9. Communicate well with patients, staff, providers, and management.
10. Fill allergy test trays as needed and assist with the oversight of antigens and other immunotherapy department inventory.
11. Monitor for non-compliance and follow protocols for non-compliant patients.
12. Provide patient education as needed and appropriate.
13. Pull medication samples as prescribed.
14. Perform allergy lab related phone triage duties, documenting calls and messages.
