The primary goal is to increase the efficiency and productivity of the physician. It will be the responsibility of the Medical Scribe to annotate any dictated or written information for the treatment of patients following all local, state and federal guidelines for documentation into the electronic health record system. The Medical Scribe will be responsible for clearly recording each patient's symptoms, history, physical exam and documenting the diagnosis. This position will have no direct involvement in patient care but has an interdepartmental role in assisting nursing and front office staff.JOB DUTIES AND RESPONSIBILITIES The primary role of the Medical Scribe is to assist the clinician with documentation of each patient's medical chart during his or her visit. Accompany the clinician into the patient examination area to transcribe a history and physical examination as given by the patient and physician. Document any procedures performed by the clinician or nurses, on the electronic medical chart. Is an expert in the use of various templates and documentation techniques available in the EHR system and uses these tools efficiently so individual encounters can be well documented, as well as accurately congregate data collected for statistical reporting. Transcribes any consultations or discussions with family members and/or the clinician. Completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data to get the patient's workup complete so that the clinician is able to make his or her decisions regarding that patient. Orders in-house testing, Immunizations, lab testing, diagnostic testing along with generating referrals. KNOWLEDGE, SKILLS, AND ABILITIES Recognition of physical exam process and ability to record exam details quickly and accurately. Must be able to listen to complex medical information and summarize in a clear, complete, and concise fashion. Strong attention to detail. Computer proficiency in Windows environment and ability to quickly learn new applications. Ability to communicate effectively in the English language in person, by phone and in writing. Ability to type 45 WPM or greater EDUCATION & EXPERIENCE High school diploma or equivalent. One year experience in healthcare field or minimum 2 years of college education. PREFERRED QUALIFICATIONS 2-4 years of college education desired with Medical Terminology, Anatomy and Physiology coursework/degree. Experience working in an outpatient clinic or doctor's office. Essential Job Duties: • Welcomes patients by greeting them, in person or on the telephone, answering or referring inquiries. • Prepares patients for the health care visit by directing and/or accompanying them to the examining room; providing examination gowns and drapes; helping them to position themselves for the examination and/or treatment; arranging examining room instruments, supplies, and equipment. • Verifies patient information by interviewing patient, reviewing and/or recording medical history; taking vital signs; confirming purpose of visit or treatment. • Adhere to safety and infection control protocols. • Supports patient care delivery by helping health care providers during examinations; preparing laboratory specimens; performing basic laboratory tests on the premises; disposing of contaminated supplies; sterilizing medical instruments; administering medications on the premises; authorizing drug refills as directed; drawing blood; preparing patients for x-rays; taking electrocardiograms; removing sutures; changing dressings. • Conducting occupational health tests, including Audiometry, Spirometry, and fit testing. • Assist providers with workers' compensation injuries, administer first aid, and conduct drug screening and alcohol collection. • Maintain accurate records of all collections. • Assist in scheduling appointments and coordinating services with other healthcare providers. • Track and complete referrals in coordination with adjusters and outside vendors. • Communicate test results to patients and healthcare providers as directed. • Maintain a high level of confidentiality and proper storage of PHI. • Perform blood draws and other specimen collection procedures in accordance with established protocols and guideline. • Keeps supplies ready by inventorying stock; placing orders; verifying receipt. • Coordinate clinic logistics and equipment setup and breakdown at various work sites; travel to different work sites as required. • Perform other duties as assigned, demonstrating motivation, reliability, organization, and punctuality. • Other duties as assigned Requirements: Required Licenses and Certifications: Medical Assistant Certificate preferred. Phlebotomy certification BLS certified with CPR/AED proficiency. Education, Skills, and Qualifications: Completion of medical assistant coursework from an accredited program. High School Diploma, GED, or suitable equivalent. Prior experience in an occupational health setting preferred. Knowledge and experience with electronic medical records. Current driver license and reliable transportation. Ability to travel, as required. Physical Demands: The position requires the ability to stand for extended periods of time to perform various tasks. It also involves lifting objects weighing up to 25 pounds. Good dexterity and hand eye coordination are essential for handling equipment and performing precise tasks. The role may involve repetitive tasks, requiring the candidate to have the physical capability to carry them out effectively. Additionally, good visual acuity is necessary for reading and accurately recording information. The candidate should be able to bend, kneel, and reach to access and handle materials and equipment. Excellent mobility is needed to move around the clinic and perform examinations efficiently. The role may occasionally require working in confined spaces, which the candidate should be comfortable with.

JOB SUMMARY: Under the direction of the Director of Nursing, works as an integral part of the healthcare team, responsible for the facilitation and delivery of high quality, direct patient care. Responsible for the intake of patients for appointments, which includes verification of check-in procedures, review of medical screenings (depression, domestic violence), timely rooming, obtaining vital signs of patients (temperature, pulse, respiration, blood pressure, pain levels, oxygen saturations) ; confirms chief complaints/reason for visit ; reviews patient medical history, allergies, medications, and confirms information in Electronic Health Record (EHR). 3. Anticipates medical provider's needs, and is proficient in set-up of medical procedures, including but not limited to, performing and/or preparing ordered lab specimens, procedural tray set-up for vasectomies, circumcisions, IUD placement/removals, sutures, casts, pap and pelvic examinations. 4. Responsible for data entry of patient information for compliance with quality measures (Quality Improvement Process (QIP); Government Performance and Results Act (GPRA), and Partnership Measures (as applicable) for facility reimbursement. 5. Assists with patient flow management- acts as a liaison to inform patients of current status, wait time, relays pertinent information to providers and is responsible to facilitate follow-up scheduling of appointments and/or rescheduling as needed. 6. Participates in patient education, ensuring the patient is provided resources to enhance understanding of medications, treatments, disease management, expectations and when to follow-up if needed. Assist with providing explanation of treatments and procedures at the start of applicable appointments. 7. Proficient and responsible for the delivery of routine immunizations, including the ability to perform rights of medication administration (right drug, dose, route, reason, patient, time, documentation), the ability to identify appropriate injection sites (adult / well-child); and adhere to antiseptic technique with medication preparation and delivery.
