Staff Nurse or Nurse Practitioner, Nurse Educator Diabetes Key Responsibilities: Coordinating patient self-management initiatives in a family centric program for pediatric and adolescent patients in Boston Children's Hospital's Division of Endocrinology's ADA recognized outpatient diabetes self-management education program. The program uses a multi-disciplinary approach in the management of children and adolescents with diabetes (Type 1, Type 2, steroid induced and cystic fibrosis related) and their families. Working with patients diagnosed with diabetes and their families, to facilitate improving self-management age specific goals for all patients; providing lifestyle coaching, and conducting one-on-one patient visits and group visits in a hospital clinic setting. Providing patient and family education; collaborating with outside community resources including but not limited to school and insurer-based case coordination services. Providing case management including oversight and interpretation of laboratory results for patients with diabetes; ordering of therapeutics, durable medical supplies and refills for prescription medications; as well as coordination of referrals and diagnostic studies as needed in consultation with the patient's physician. Temporary Nurse Practitioner, Department of Youth Services (DYS) Program *Please note this role is temporary and is expected to last for approximately 6 months. Key Responsibilities: Providing care to adolescent males and females in an NP-led practice, located within DYS facilities, which are part of Boston Children's Hospital Adolescent Division. Nurse Practitioner - Complex Care Services Advanced Practice Registered Nurse (APRN) to function in the inpatient Complex Care Service (CCS) at Boston Children's Hospital. CCS is a long-standing and nationally regarded inpatient service in Pediatric Hospital Medicine within the Division of General Pediatrics that serves hospitalized children with medical complexity, who have complex chronic conditions and functional limitations. Common reasons for admission to CCS include infections (aspiration pneumonia, pyelonephritis), chronic condition exacerbations (epilepsy, digestive dysmotility), and post-operative recovery following surgery (baclofen pump placement, gastrostomy, spinal fusion, etc.). PACT (Pediatric Advanced Care Team) Nurse Practitioner Fellowship PACT offers a full-time, 12-month, Pediatric Nurse Practitioner Fellowship. The pediatric nurse practitioner fellow will train alongside two physician fellows and one social work fellow in this distinctive interprofessional program. The fellowship includes mentored clinical supervision on PACT across settings (inpatient, outpatient, community, bereavement) in the care of children with serious illness and their families in the domains of communication, pain and symptom management, psychosocial assessment and management, advance care planning, interdisciplinary collaboration, and various delivery models in palliative care.
