Lutheran Social Services of Central Ohio is currently seeking a Case Manager for Support Services for Veteran Families (SSVF) for Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the SSVF Case Manager with Lutheran Social Services?
Work closely with community and government agencies (social services, shelters, faith and community based organizations, prisons, jails, VA facilities, VA grant and per diem programs, legal aid, settlement houses, landlords, etc) to identify very low-income veteran families who are at risk of homelessness.
Using appropriate tools, screen identified families for eligibility, prioritize those families in greatest need and assess whether other interventions may better serve the veteran family being screened.
Perform intake for new participants, including needs assessment, and mental/physical health screenings.
Refer ineligible families to alternate resources/supports as available in the community or through the VA.
Work closely with clients to develop and implement participant specific, individualized, intentional action-oriented goal plans based on the CTI model, with a focus on achievement of housing stability.
Implement planning systems that reflect client strengths and needs, client responsibilities, Case Manager responsibilities, and action steps to help clients in achieving their goals.
Provide budgeting and other planning assistance toward the goal of housing retention.
Coordinate with VA services, veteran programs, Faith Mission Resource Specialists and other community resources to assist clients in seeking income, employment and/or other needed supports.
Link clients to internal resources as appropriate for financial supportive services in the form of direct client assistance payments to third party (e.g., landlords).
Refer clients to partner agencies and community resources that benefit their short-and long-term goals.
Interface closely with all Faith Mission staff, VA staff and public assistance agencies to provide quality wrap-around services and access to benefits for clients.
Gather information on potential resources for clients in the larger community, creating working relationships and disseminating information for immediate and ongoing support to client.
Maintain clear and precise data and case notes, including timely submission of any CQI reports and careful documentation of all client interactions.
Actively participate in weekly Case Review, working with colleagues toward positive client outcomes.
Work with client to maintain on-going support relationships with community resources, family and friends.
Successfully complete the Critical Time Intervention training.
Contribute to an atmosphere of safety, sanctuary, dignity, support and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
Assist with crisis intervention and de-escalation by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.

**Responsibilities**
The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. What We're Looking For
Education:
Qualified Behavioral Health Specialist I - High Diploma + 3 years of experience ($15-25)
Qualified Behavioral Health Specialist II - Bachelor's degree in psychology, counseling, rehab counseling, or a related field with at least two years of related experience preferred. ($18-28)
Qualified Behavioral Health Specialist III - Master's degree in psychology, school psychology, counseling, vocational rehabilitation, or a closely related field is highly preferred. ($25-35)
Experience: Community mental health or social services experience strongly preferred.
Training: Must be able to complete CPR, First Aid, and de-escalation training.
Requirements:

Key Responsibilities:Leadership & Oversight

Quality, Compliance, & Risk Management

- Contribute to quality assurance efforts, critical incident reviews, and continuous improvement initiatives across service lines.
- Serve as a liaison with external medical providers, hospitals, and regulatory agencies.
Education & Consultation
- Provide training and consultation to staff on topics such as psychotropic medications, mental health diagnoses, crisis de-escalation, and medical conditions common in IDD populations.
- Stay current on emerging best practices and innovations in behavioral health and developmental medicine.
Required Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) with active licensure in the State of Ohio
- Board certified or board eligible in Psychiatry; additional certification in Child/Adolescent Psychiatry or Addiction Medicine is a plus
- Minimum 3-5 years' experience working with individuals with IDD and/or behavioral health conditions in community-based or residential settings
- Strong understanding of person-centered, trauma-informed, and recovery-oriented care models
- Familiarity with DODD, Medicaid, and nonprofit healthcare systems
- Excellent leadership, communication, and organizational skills
Preferred Qualifications:
- Prior experience in a Medical Director or supervisory role

Qualifications
* License or Certification:

Must meet eligibility requirements for CCM or ACM certification upon entry into this position OR within two years of entry into the position.

* Minimum Qualifications:

POSITION SUMMARY
Community Health and Wellness Partners (CHWP) is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. The Team Care Case Manager provides leadership by maintaining an excellent attitude and utilizing leadership by example. As part of this model, the Team Care Case Manager provides relevant interventions under the guidance and supervision of the Assistant Behavioral Health Medical Officer and Behavioral Health Manager and is the primary interface for patients to access community resource services. Responsible for the coordination of ancillary services, community resources, screening for chronic conditions and resource needs, providing brief intervention, referral to appropriate treatment and services, and documentation of care provided. The Team Care Case Manager is also responsible for being a liaison for community agencies as assigned. Supports the coordination activities of all team members engaged in clinical care for patients. Responsible for supporting clinical staff in team-based care and encourages team members to stay focused on the organization's Mission, Vision, and Values.
PRIMARY ACCOUNTABILITIES

Ensure accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as assigned and are always maintained current and relevant.

Promote and exemplify excellent customer service to ensure that all clients/patients are served with a high level of customer satisfaction and care.
Operational Excellence
Ensure the delivery of all coordination and care is consistent with all regulatory, accreditation and professional standards, including behavioral health policies and quality initiatives.
Understand and ensure all practices and procedures related to the delivery of behavioral health and clinical services are consistent with contracted protocols and procedures, regulating entities (HRSA, HIPAA, TJC, PCMH, etc.) professional standards, and that the team-based care model is followed with efficiency.
Relationship Management
Develop and ensure favorable relationships with all team members, patients, and their families. Achieve commitments from patients to adhere to an effective plan of care, and ensure all patient services are delivered in a caring and professional manner.
Develop and ensure ongoing, positive relationships and collaboration between members of the team assigned. Ensure the success of team-based care and that relationships with all clinic operations staff are highly respected.

PRIMARY TASKS & DUTIES
Participates as a member of the team-based care model by being a strong team builder, working to support performance goals related to patient care.
Administers clinically indicated standard screening and outcome measures instruments (exp. PHQ-9).
Provides direct behavioral health interventions based on screening outcomes and patient symptoms to patients as indicated and qualified to perform with excellence.
Will assist in coordinating ongoing care for patients who are at risk for poor health outcomes.
Perform comprehensive bio-psychosocial assessment. Conduct follow-up as appropriate based on results of evaluation.
Responsible for coordinating referrals for Home Health/Hospice, Long Term Care Facilities, and other social services available in the patient's community.
Assist patients with determining eligibility for health coverage programs and other programs as needed. This includes assistance with forms, making initial contacts and appointments for the patient. Follow-up with the patient to ensure linkages to services are made.
Supports patient practice of skills learned in consultation with Primary Care Providers, Behavioral Health Providers, Behavioral Health Psychiatric Providers, and other ancillary providers.
Supports collaboration of primary care teams and behavioral health staff regarding symptom management.
Maintains up-to-date contact information for commonly used community resources.
Develop and maintain collaborative working relationships with community providers of services.
Assists in delivery of group visits, as assigned.
Is accessible in person, over the phone, or via telehealth technology by being present and available to CHWP clinical teams during clinic hours.
Participates in quality strategies to evaluate compliance with standards and identify opportunities to improve patient outcomes.
Aids Behavioral Health Manager in capturing program evaluation and fidelity measures.
Attends all meetings, patient conferences, planning sessions related to quality assurance, patient care and other related topics within the health center as assigned.
Participates in continuing education to maintain best practices.

WORK ENVIRONMENT
The work environment characteristics are those for a normal community health center environment (ambulatory primary care office). Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may require occasional Saturday and/or evening hours.
Work hours
Full-time, non-exempt hourly position. Generally, a 40-hour work week is required for full-time; part-time as agreed upon by parties; hours are subject to change. Office hours vary by site Monday through Friday. Some Saturdays and after business hours work may be required. Assigned meetings and travel are a part of the position. Remote work /Telecommuting may be available on a limited basis at the discretion of the Integrated Health Coordinator or Manager. Paid Time Off (up to 150 hours 1st year), six paid holidays off while offices are closed, and birthday/personal day off.
Wages
Starting wages for Team Care Case Manager will be negotiable based on Community Health & Wellness Partners pay policies and factors such as education and experience, national and state average compensation recommendations based on location and region. Benefits begin on date of hire. We offer medical, vision, dental, life, LTD and many other voluntary benefits to choose from.
Introductory Period
Ninety (90) calendar days probationary period. The successful completion of the introductory period does not alter the at-will employment status.

Qualifications
QUALIFICATIONS AND REQUIREMENTS
Education
Associate's degree in social work or related field, preferred.
One (1) to three (3) years of experience in behavioral health or integrated primary care practice.
Certificates and Licenses
Valid Ohio driver's license.
Active Social Work or Counselor License within the State of Ohio, preferred.
Professional
Demonstrates expertise in relevant behavioral health practices, protocol, trends and best practices in clinical areas assigned.
Demonstrates knowledge and success in effecting overall clinical operations.
Experience/Knowledge of accreditation processes and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment.
Ability to manage multiple responsibilities and emergency situations successfully.
Proficiency in knowledge of office management, use of computer, software packages, and office machines.
Physical/Environmental
Occasional lifting of 40 - 50 lbs. and pushing of 5-20 lbs.

Initial Client Home Evaluations

Nurses counsel clients and their families regarding self-care, chronic condition management, and evaluate clients responses to care provided.
Hospital Relations
Nurses act as the clinical care coordinator for assigned individuals and their families, collaborating with clients other healthcare professionals as needed. Registered nurses may also initiate referrals as necessary. RNs are a very important part of a ComForCare franchisees growth, as they form the cornerstone of the new client processing and outreach systems.
Working with Caregivers
After doing initial client home evaluations, RNs may help select an appropriate caregiver whose personality and skills are compatible with the needs and persona of the client. RNs will review care plans with caregivers to ensure understanding and help clients to receive the best care possible. RNs may also help to train and instruct caregivers on critical skills that may be necessary when assisting with condition management.

RNs generally do not have to participate in many front-line nursing activities, acting mainly as a position of authority and performing administrative-style duties.

What are we looking for?
You must have current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing within state of employment.
You will have thorough working knowledge of current care standards and regulations.
You will have comprehensive working knowledge of current medication regulation and law
You will have knowledge of the requirements for providing care and supervision appropriate to the residents.
You may have experience in hands-on care of memory impaired residents is preferred, but not required.
You may have experience with an Electronic Medical Record (EMR) as it is preferred, but not required.
You can read, write, understand, and communicate in English with our Residents!
You will have a positive and energetic attitude who will LOVE our Residents!
You must be active as this role requires standing, walking, bending, kneeling, and stooping all day.
You must have the ability to frequently lift and/or move items up to 50 pounds and perform two-person transfers.
You must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.
You must be criminally cleared.
