Position summary: The Hospice LPN is a licensed caregiver who provides nursing care under the direction and supervision of the Registered Nurse. Participates in the nursing process in the planning, implementation and evaluation of care in accordance with patient care standards and physician orders. He/she acts as a liaison between the physician, patient and family and collaborates with other members of the interdisciplinary team. He/she assists in the orientation process of new personnel, and may participate in Health Center and department committees. LPN must be able to demonstrate the knowledge and skills necessary to provide care appropriate to age groups according to specific chronological age, developmental, age, and/or psycho-social maturity.
Qualification:
Education: Graduate of accredited LPN school of Nursing.
Required length and type of experience: Previous experience in specific nursing area preferred. IV and Antibiotic certification required.
Required licensure, certification or registry: Current license by the Ohio State Board of Nursing

2D Acute Rehab (2DAR) is a 15 bed rehab unit that cares for patients with hip fractures, CVAs (Cerebrovascular Accident) and other debilities. Patients participate in 3 hours of therapy 5 days a week. They welcome new grads, and have no call or rotating shifts.

Job Description
The General Observation Nurse plays a key role in monitoring physical conditions of patients, providing basic nursing care, and assisting in the early detection of potential health issues. This role requires collaboration with healthcare team members to ensure effective patient management.
Responsibilities
Continuously monitor vital signs and document changes in patient conditions.
Administer medications and provide assistance with daily living activities.
Communicate effectively with healthcare team regarding patient care plans.
Recognize and respond to changes in patient conditions requiring urgent care.
Adhere to infection control policies and ensure a safe environment.
Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
Required Experience / Certifications / Licensure
Minimum 1 year of nursing experience in a relevant specialty area.
Current Ohio nursing license.
BLS certification from the American Heart Association.

Southwest General's Float Pool team provides supplemental staffing for all medical/surgical areas in the hospital, and occasionally to specialty units as needed (but will only function within their demonstrated level of competency). No call and no rotating shifts.
HOURS:
Full-time, day shifts. 7:00am-7:30pm with rotating weekends.
