Job Summary
Directly responsible and accountable for performing chart reviews, physician education, and development of tools to ensure that our provider partners are compliant with Risk Adjustment. Provide overall coding expertise as well as administrative and technical oversight to ensure successful integration of Molina Medicare's Risk Adjustment initiatives. May require some travel to various provider partner locations
• Performs on-going chart reviews and abstracts diagnoses codes under the HCC Model.
• Develop an understanding of current billing practices in provider offices to ensure that diagnoses codes are submitted accordingly.
• Documents results/findings from chart reviews and provides feedback to management, providers, and office staff.
• Responsible for administrative duties such as planning, scheduling of chart reviews, obtaining of medical records, and provider training and education.
• Monitor HCC Coding Accuracy at various levels of detail (e.g., by state, by product, by demographic segmentations). Extract information necessary to identify where there are low performing physicians; follow up with plan for education and training. Continue to audit to ensure training is implemented.
• Resolve and track escalated issues. Track any coding issues identified either at the provider level (including Molina sites) or vendor; manage any non-compliance situation or potential fraud or abuse.
• Utilize discretion and autonomy to select provider for further training or audits; coordinate efforts with internal clients such as Coding Manager, RAMP Director, State Medicare Directors and Provider Services.
• Determine coding quality as it relates to CMS standards; selects physicians or vendors that require an audit.

Key Responsibilities / Essential Functions
Assigned Release of Information request types will primarily be Continuing Care and Disability Determination Services, with cross-training on other request types as supervisor deems appropriate based on experience and performance
Accesses Release of Information requests and medical records for healthcare client(s) according to the specific procedure and security protocol for each client
Completes Release of Information requests daily, prioritizing requests as needed based on turnaround timeframes and procedures of VRC and the service agreement between the healthcare facility and VRC
validates requests and signed patient authorizations for compliance with HIPAA, other applicable federal and state statutes, and established procedure
classifies request type correctly
logs request into ROI software
retrieves and uploads requested portions of the patient's medical chart (from electronic or physical repository)
performs Quality Control checks to ensure accuracy of the release and to avoid breaches of Protected Health Information (PHI)
checks for accurate invoicing and adjusts invoice as needed
releases request to the valid requesting entity
Rejects requests for records that are not HIPAA-compliant or otherwise valid
For records pulled from a physical repository, returns records to proper location per VRC and healthcare client procedure
Documents in ROI software all exceptions, communications, and other relevant information related to a request
Alerts supervisor to any questionable or unusual requests or communications
Alerts supervisor to any discovered or suspected breaches immediately
Alerts supervisor to any issues that will delay the timely release of records
Answers requestor inquiries about a request in an informative, respectful, efficient manner
Stores all records and files properly and securely before leaving work area.
Ensures adequate office supplies available to carry out tasks as soon as they arise
Is available and knowledgeable to take on additional healthcare facilities or request types to assist during backlogs

They also provide infusion therapy, transfusions, injections, and central line care. SUMMARY:
Performs basic and advanced Polysomnography and related tasks. Scores and analyzes Polysomnographic data. Demonstrates knowledge and skills necessary to provide care to pediatric, adult, and geriatric patients.
ESSENTIAL FUNCTIONS:
5% - Calibrates all equipment necessary for PSG recording including oximeter, polygraph, CO2 monitor, etc.
5% - Interacts with patients to obtain vital signs, pertinent medical sleep history, and responds to basic patient needs and requests. Provides basic information to patients on equipment and recording procedures.
20% - Attaches electrodes and other recording devices to patients undergoing sleep studies according to standardized procedures. Removes and cleans all such devices at the completion of sleep study.
30% - Monitors PSG collection computers and make appropriate adjustments to produce quality PSC recordings free of artifact. Records observations of patient activities and technologist interventions. Able to troubleshoot as necessary to resolve problems with equipment/recordings.
10% - Scores sleep records for sleep stages, respiratory events, limb movements, and other parameters. Meets department standards for interscorer reliability.
20% - Titrates PAP devices and O2 during sleep study for optimal resolution of respiratory events and nocturnal desaturations. Conducts CPAP training and desensitization prior to start of CPAP study.
3% - Records written documentation of patient activity in patient's Sleep Center chart or electronic collection file.
2% - Maintains clean and orderly work area and cleans equipment per department policies. Replenishes supplies used in all phases of sleep study as needed.
5% - Stays current in the field of sleep technology by participation in the continuing education activities required for AASM accreditation.

Position Summary:
Responsible for performing a variety of duties including but not limited to: greeting patients, vendors, and other visitors and notifying appropriate staff of their arrival, operating telephone console to route incoming calls and place outgoing calls, scheduling of patient appointments, obtaining current and accurate insurance information each visit, verifying coverage through insurance company, obtaining pre-certifications or prior authorizations as needed, maintaining daily accurate records for timely billing of accounts, notifying patients of account status and payments due using collection techniques to assist in keeping accounts receivable current. Assigned to a specific practice but may need to assist in covering other practice sites.
What you will do:
* Obtains, verifies, updates and scans patient information and provides support services to patients and medical staff
* Verifies patient's current insurance coverage through insurance websites
* Collects and posts co-pays and past due balances, and completes any necessary forms obtaining signatures as necessary
* Obtains prior authorizations and pre-certifications as needed
* Manages referrals through EMR
* Follows all incentive guidelines by helping the practice meet a variety of incentive program goals, including Meaningful Use and Patient Centered Medical Home
* Performs daily banking through Virtual Merchant and FRMC cashiers
* Prepares a daily financial spreadsheet for practice
* Ensure the submission of timely, accurate and complete information to the Central Billing Office
* Uses collection management reports to keep accounts receivable current
* Looks for instances of Red Flag Identity theft patterns and reports potential threats to Privacy Officer
* Requests, locates, sends and receives patient medical records according to federal, state or local guidelines
* Maintains medical appointments for patients
* Answers phones in a pleasant manner, screens calls following clinic guidelines to appropriately and expeditiously direct caller
* Maintains clean, orderly waiting area including patient message board, children's area, and reading materials
* Organizes and maintains an inventory system to ensure adequate levels of supplies with emphasis on patient care and cost containment

What you will need:
* High school graduate; vocational education preferred.
* Experience in medical ambulatory setting.
* Current CPR certification or obtained within 3 months of employment and maintained.
* Proficient in the use of personal computers with strong typing skills. Skilled in the use of Microsoft Outlook, Excel, and Word is desired.
* Eligibility: All employees must be able to provide proof of their identity and their right to work in the United States. As a condition of employment, employee will have annual tuberculin testing. Initial testing upon hire will consist of a 2-step tuberculin test, symptom survey, or chest x-ray. Employees can elect to have the Hepatitis vaccine or sign a wavier to decline due to prior vaccination.
* Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.

Toledo Clinic's Internal Medicine Office, located in Perrysburg, Ohio, is seeking a full-time Medical Receptionist. This position will work Monday - Friday (6:45a-3:00p). Employee benefits are offered and there are no weekends or holidays.
Perform various clerical duties to support the operation of the office.
Works under the supervision of the Physician/Office Coordinator.
Principal Duties & Responsibilities:

1) Answering phones - courteously and professionally.

3) Verifying referrals and/or pre-certifications.
4) Updating insurance information.
