Visits include all areas serviced by VNS Health Plans including upstate and downstate counties
• Performs in-home assessment on members to determine the appropriate service plan, including completion of UAS assessment questionnaire, tasking tool, and a projected service plan. Visits include all areas serviced by VNS Health Plans
• Explains VNS Health Plan benefits, including an explanation of the member's handbook
• Ensures compliance with state and federal regulatory standards and VNS Health Plans policies and procedures
• Identifies opportunities for alternative care options and contributes to the development of a safe member centered service plan
• Consult with supervisor and others in overcoming barriers in meeting goals and objectives
• Maintains current knowledge of organizational or state-wide trends that affect member eligibility
• Coordinates with other departments, e.g. Care Management, Legal Affairs, Grievance and Appeals, Compliance, Membership Eligibility Unit, Quality as needed
• Participates in requests for out-of-network services when a member receives services outside of VNS Health Plans network services
• Keeps current with all health plan changes and updates through on-going training, coaching and educational materials

Qualifications
Licenses and Certifications:
Current license to practice as a Registered Professional Nurse in New York State required. Certified Case Manager preferred.
Education:
Bachelor's Degree in nursing or equivalent work experience required.
Master's Degree in nursing or equivalent work experience preferred.
Work Experience:
Minimum two years of clinical assessment, homecare or hospital experience required.

Regional Medical Director
JOB SUMMARY:
The Regional Medical Director (RMD) provides strategic clinical leadership and oversight for multiple BoldAge PACE sites across a geographic area that can include several states, maintaining active medical licensure in each applicable state BF1 . Regional Medical Directors will be licensed in multiple Regions to allow for coverage for other RMD's vacations and in emergency situations. Working in partnership with the Chief Clinical Officer, th e RMD ensures BoldAge Pace consistently delivers exceptional participant outcomes, regulatory compliance and operational efficiency. The Regional Medical Director role combines direct participant care with a local panel of patients at the provider's home office BF2 with broader regional leadership responsibilities, including the ability to provide coverage for other regional locations including but not limited to fulfilling needs related to APP practice BF3 , in an emergency or when state regulations require active patient practice. In addition to maintaining a clinical panel, the Regional Medical Director provides administrative leadership by overseeing the Regional Quality Assessment and Performance Improvement (QAPI) program, collaborating with national clinical leadership to develop and maintain medical policies, guidelines, standing order protocols, and ensuring the efficient use of resources to achieve program goals. This position provides direct guidance and supervision to primary care physicians, advanced practice providers and other providers as assigned. ESSESNTIAL DUTIES AND RESPONSIBILITIES :
Clinical Leadership & Oversight
Carry a panel of participants, providing direct care in region (center, home and facility setting), including, but not limited to, taking on-call shifts (national coverage), providing emergency coverage in the event of provider absence and interdisciplinary team (IDT) participation.
Oversee and evaluate participant care delivery 24/7 through established clinical leadership structures.
Ensure timely completion of participant assessments and individualized care plans in compliance with PACE requirements.
Collaborate with site Medical Directors and interdisciplinary teams to ensure participant needs are met and outcomes optimized.
Collaborate and participate in national Clinical Leadership Meetings (led by CCO), acting as the regional representative and expert. Provide regional expertise for national clinical and operational development and represent regional needs at the national level.

EXPERIENCE EDUCATION AND CERTIFICATIONS:
M.D. or D.O. with current medical license and the ability to obtain and maintain active licensure in all states in which BoldAge PACE operates. Must also hold current DEA registration and have the ability to obtain and maintain staff privileges at PACE-contracted agencies. Board certification in Internal Medicine or Family Practice required; advanced certification in Geriatrics preferred.
Demonstrated experience in a managed care environment, collaborating with peers and other healthcare providers to address utilization management, quality management, performance improvement, pharmacy and therapeutics, peer review, credentialing, and physician leadership matters.
Minimum of 3 years in a lead administrative role with responsibility for clinical oversight across multiple locations or programs.
A minimum of 3 years' experience working with frail and elderly populations in acute care, primary care, long-term care, or community-based settings.

REPORTS TO: Chief Medical Officer
SUPERVISES: Providers
PURPOSE: Direct patient care, provide clinical supervision, and carry out program development.
ESSENTIAL FUNCTIONS:
Direct Patient Care:
- Medical screening, evaluations and primary care to ACS adolescents.
- Serve as liaison between Hospital, ACS, and other medical facilities for patients referred to those facilities.
- Participate in case conference and case management activities.
Clinical Supervision:
- Recruit, train and supervise medical and dental providers in the performance of their clinical duties.
- Inspect, correct, and process assigned bi-weekly assigned employee timesheets.
- Participate in quality improvement activities, in conjunction with the Quality Improvement Team and the Chief Medical Officer.
- Provide in-service training for the Hospital's professional and administrative staff.
- Identify and maintain clinical staffing requirements.
- Review and modify protocols periodically and evaluate staff performance in relation to these protocols.
- Coordinate with hospitals regarding patients who are transferred to the emergency room or admitted to the hospital.
- Manage dental services and contractual requirements.
- Coordinate with OCFS/CTH regarding patients who have chronic medical problems and are transferred.
- Assure that the mid-level practitioner(s) provides specified services or clinical activities in accordance with accepted medical standards.
- Observe, implement, and promote the federal regulations as defined by HIPPA and Policies and procedures as defined by the Hospital.
- Prepare monthly goals report for submission to ACS.
- Ensure compliance with all grant specific reporting and requirements.
- May initiate and participate in research related to health care or allied fields, in collaboration with the Chief Medical Officer.
Assignment, Review And Approval Of Work:
The Program Director may assign other duties, assignments, and special projects, as needed.
Environment:
The office is temperature-controlled and the noise level is kept to a minimum in accordance with professional office practices. The incumbent will have office space to conduct his/her job functions. The agency observes a no-smoking policy.
Background and Education:
M.D or D.O. degree from an accredited medical school; New York State Board Certification in Pediatrics or Family Medicine, M.P.H. or experience in hospital administration recommended; minimum three years related experience in family, pediatric, adolescent urban populations; ability to speak Spanish or other languages a plus.
