Required Qualifications:

Major Duties and Responsibilities:

Supply Distribution: Collect, fill, and deliver supply requisitions to designated units while ensuring smooth daily operations.
Records Management: Organize, file, and maintain resident health information manually and electronically, ensuring records are complete and accurately assembled.
Compliance and Privacy: Safeguard health information in accordance with established policies, procedures, and privacy regulations.
Information Retrieval and Communication: Retrieve and deliver records as needed, assist with inquiries, and prepare documentation for insurance, Medicare, Medicaid, and other stakeholders.

The Patient Services Coordinator-LPN is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day-to-day office and staff management
Manages schedules for all patients. Edits schedule for agents calling in sick, ensuring patients are reassigned timely. Updates agent unavailability in worker console.
Initiates infection control forms as needed, sends the HRD the completed “Employee Infection Report” to upload in the worker console.
Serves as back up during the lunch hour and other busy times including receiving calls from the field staff and assisting with weekly case conferences. Refers clinical questions to Branch Director as necessary.
Maintains the client hospitalization log, including entering coordination notes, and sending electronic log to all office, field, and sales staff.
Completes requested schedule as task appears on the action screen. Ensures staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.
Completes requested schedules for all add-ons and applicable orders:
Schedules discharge visit / OASIS Collection or recert visit following case conference when task appears on action screen.
Schedules TIF OASIS collection visits and deletes remaining schedule.
Reschedules declined or missed (if appropriate) visits.
Processes reassigned and rescheduled visits.
Ensures supervisory visits are scheduled.
Runs all scheduling reports including Agent Summary Report and Missed Visits Done on Paper Report.
Prepares weekly Agent Schedules. Performs initial review of weekly schedule for productivity / geographic issues and forwards schedule to Branch Director for approval prior to distribution to staff.
Verifies visit paper notes in scheduling console as needed.
Assists with internal transfer of patients between branch offices.
If clinical, receives lab reports and assesses for normality, fax a copy of lab to doctor, make a copy for the Case Manager, and route to MedicalRecords Department. Initiate Employee / Patient Infection Reports as necessary.
If clinical, may be required to perform patient visits and / or participate in on-call rotation.

Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which he / she practices
Have at least 1 year of home health experience.

Must be currently licensed in the State of employment if applicable.

This job is a full-time, benefited position at Summit Spine & Joint Centers that reports to the Administrative Manager. This position's primary location will be at the Administrative Building in Lawrenceville, GA.
Responsibilities:
Assisting patients with questions about their medications
Promoting medication compliance among patients
Coordinating patients' medication orders
Updating patient information on electronic medicalrecords system
Communicate with various administrative teams to ensure the proper flow of information to support patient care
Following all company policies on patient services standards
Be a part of an administrative, pharmaceutical oriented back-office team
Skills And Abilities:
Must be personable and detail oriented as a representative of the practice while callers rely on proper information
Excellent verbal and written skills for proper documentation of encounters.

Education And Experience:
Background in using EMR software required

Summary Description:
The Cardiac Management Solutions division of ZOLL, manufacturer of the LifeVest , is seeking a Patient Service Representative (PSR) in an independent contractor role to train patients on the use and care of LifeVest .
LifeVest is worn by patients at risk for sudden cardiac arrest (SCA), providing protection during their changing condition. It is lightweight and easy to wear, allowing patients to return to their common activities of daily living, while having the peace of mind that they are protected from SCA.
This is the perfect opportunity for the health professional with patient care and teaching experience to supplement his or her income. A typical LifeVest patient is in the hospital awaiting discharge, and the Patient Service Representative sets up the equipment and trains the patient and caregivers on the use and care of the device. They also follow up with patients as needed to assure patient understanding and satisfaction. PSRs retain the flexibility to accept or decline assignments as their schedules dictate. Most assignments can be scheduled during free time - before/after work, and on days off.
Responsibilities:
Contact caregivers and family to schedule services
Willingness to accept assignments which could include daytime, evenings, and/or weekends.
Travel to patient's homes and health care facilities to provide services
Train the patient and other caregivers of patient (if applicable) in the use of LifeVest
Program LifeVest according to the prescribing physician's orders
Measure the patient and determine correct garment size
Review with patient, and have patient sign, all necessary paperwork applicable to the service.
Transmit signed copy of the Patient Agreement and WEAR Checklist to ZOLL within 24 hours of the assignment
Manage device and garment inventory
Disclose family relationship with any potential referral source
Qualifications:
Have 1 year patient care experience
Patient experience must be in a paid professional environment (not family caregiver)
Patient experience must be documented on resume
Completion of background check
Florida and Ohio candidates must complete a level 2 screening, fee not paid by ZOLL
Disclosure of personal NPI number (if applicable)
Valid driver's license and car insurance and/or valid state ID
Willingness to pay a $30 annual DME fee which is deducted from a completed Work Order
Willingness to pay for additional vendor credentialing (i.e. RepTrax) if needed geographically
