JOB SUMMARY: Reviews medical records after discharge for completeness in compliance with the hospitals bylaws, federal and state requirements. Assures that patient's abstracts are correctly entered into the chart deficiency system. Monitors accounts being admitted daily for duplication of medical records numbers, along with checking patients discharge status when assembling the chart. Ensures that all transcribed reports are filed in a timely manner in the charts. Communicates with physicians about delinquent reports. Accountable for the accurate transcription of dictated reports. Assures the completion of incomplete medical records within the time frame as specified by the medical staff rules and regulations. Is familiar with requirements of the state guidelines of timeframes for processing of the charts such as the History and Physical and Operatives report timeframes allowed.
