WHAT YOU WILL DO The MedicalSocialWorker utilizes social work expertise and clinical knowledge to educate patients on the best choices for treatment, health, and wellness when living with kidney disease to enhance quality of life. The MedicalSocialWorker is responsible for supporting the social and emotional well-being of patients at Satellite Healthcare. The SocialWorker conducts psychosocial assessments, participates in care planning and team review of the patient's current psychosocial needs, and provides management of services to patients to assist them in coping with issues associated with chronic kidney disease and dialysis treatments.

Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable

ESSENTIAL JOB FUNCTIONS:
Conducts psychosocial assessments to determine PACE Participants' care needs, preferences, and goals.
Participates as an active member of the IDT for Participant care planning.
Documents accurately and thoroughly in the Participants' electronic health record in accordance with industry and departmental standards including time requirements.
Communicates effectively and openly with other Interdisciplinary Team members on participant status and needs.
Maintains current and thorough written case management records, including periodic reassessments of program Participants.
Provides supportive counseling to Participants and/or their family members, as indicated.

May provide after-hours on-call coverage to support PACE participants when the PACE Center is closed as needed for support.

Program and Role Summary: Our statewide Foster Care and Adoption programs match children and youth who have been displaced from their homes due to abuse or neglect with families that can provide safe, caring homes. Wayfinder recruits, trains and certifies resource (foster) families, some of whom ultimately adopt the children they foster. We emphasize a child achieving permanency, which can be reunification with birth parents, living with relatives, adoption or guardianship. The primary focus of the SocialWorker II is to be responsible for conducting resource family assessments for relative and non-relative families, completing reports approving or denying the family, or providing case management services to the family and youth until the case closes to reunification, guardianship, or adoption. Socialworkers may work in both roles simultaneously as needed. San Mateo County Health's Aging and Disability Services Division is seeking qualified individuals for the unclassified and limited-term positions of In-Home Supportive Services SocialWorker I/II/III. SocialWorkers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
Currently, there are three limited-term vacancies and an unclassified vacancy in the In-Home Supportive Services (IHSS) Program. The program provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The SocialWorker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. Program and Role Summary
Wayfinder's Contra Costa Psycho Social Assessment program assesses resource families, including adoptive families, by completing resource family evaluations within county timeframes.
The primary focus on the SocialWorker, is to conduct resource family assessments for relative and non-relative families; and write the assessment in the county contract's allotted time. Psychosocial Assessment (PSA) SocialWorkers may also assist with finalizing the adoption for these families.
This is a part-time position budgeted for 29 hours/week with a varied schedule (weekend and evening availability, as needed). The PSA SocialWorker will provide support to both Contra Costa and Sonoma Counties, as needed.
Primary Responsibilities:
Assesses county-referred applicants to become an approved Resource Family.
Conducts a home environment assessment, and psychosocial assessment, and prepare a written report. Incorporate an assessment of an applicant's mental health and substance abuse into the risk assessment.
Provides ongoing assessments of family needs and makes appropriate referrals to services such as counseling, support groups, and other applicable services and coordinates with county workers and/or other community agencies to identify assistance funding services.
Maintains accurate and thorough records of all services provided and document all assessments, meetings, decisions, and reports.
Communicates needs, services, and progress with the referring agency, service provider, and applicants.
Provides therapeutic interventions on an ongoing basis and during crises.
Other duties as outlined in the position description.
Qualifications
Education and Experience:
Master's Degree from an accredited university in social work or social welfare, marriage, family and child counseling, child psychology, child development, counseling psychology, or social psychology is required.
Bilingual (Spanish), preferred.
Valid CA Driver License with a minimum of three (3) years good driving history and must be approved by the agency's insurance carrier to drive agency vehicles.

* Refers and assists patients/families in applying for appropriate financial programs (CCS, SDI, SSI, SSD, private pensions) and legal instruments as needed.
* Screen for any barriers to care such as substance abuse, neglect, financial limitations or housing.
* Serves as a resource and provides counseling and treatment related to palliative care or end-of-life planning.
QUALIFICATIONS
Required Education: Master's degree in social work/welfare issued by a school accredited by the Counsel of Social Work Education.
