Job Summary: Licensed Medical Social Workers (LMSW) of Kindful Health are responsible for the delivery of various social work services to hospice patients and their families. Our Social Workers (LMSW) provide social service counseling and guidance to facilitate the maximum functioning and coping capacity to each patient and caregiver/family member.
Essential Education and Experience: Masters of Social Work from an accredited school of social work and holds a current license of Social Work in Texas. Minimum one (1) year experience as a social worker in long term care or medical surgical/acute care setting. One (1) year of hospice experience is preferred but not required.
Responsibilities & Duties: The LMSW will be responsible for:
Adhering to all regulatory requirements and professional standards
Assessing the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicating findings to the RN Case Manager and other members of staff including the interdisciplinary group.
Assessing the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, to include funeral arrangements if needed.
Supervising the implementation of an individualized social work plan of care and notified Bereavement Coordinator of bereavement assessment needs.
Participating in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness.
Providing consultation, education, and support to the interdisciplinary team on social work needs.
Following policies and procedures consistent with recognized standards of professional practice
Providing high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
Collaborating with your Clinical Director and entire team to provide a "concierge" level hospice experience

Compensation:
Salary - $65-75K Annually

Responsibilities
Essential Functions:
-Conduct a diagnostic assessment of mental health and/or substance use disorders utilizing the most updated Diagnostic Statistical Manual (DSM) as well as comprehensive clinical assessments and other psychometric screenings to deliver effective evidence-informed and emerging best practice clinical treatments addressing the multi-dimensional and holistic biopsychosocial- spiritual needs, strengths, and goals for the diverse breadth of patients.
-Provides trauma-informed, evidence-based, strengths-based and culturally affirming therapeutic counseling for individuals in Central Health services to enhance a patient's overall well-being. Works closely with the patient, their treatment team and care providers to develop care plans and provide services that is in the best interest of the patient and consistent with Central Health's policies, mission, and goals.
-Lead and facilitate psychoeducational and counseling groups in various clinical areas; foster group cohesion, encourage participation, and manage group dynamics effectively.
-Fosters a safe, supportive and healing environment for all patients and their identified support systems. Acts as subject matter expert on social work approach, intervention, and treatment of mental health and substance use disorders.
-Responds to mental health and substance use crisis through the provision of trauma informed crisis intervention, de-escalation, risk assessment, and other evidence-informed interventions for patients independently or in collaboration with community organizations.
-Advocates with internal and external stakeholders for continued focus on the choices, values and preferences of the patient. Fosters a safe, supportive and healing environment for all patients and their identified support systems.
-Works closely with the patient's treatment team and care providers to provide care that is in the best interest ofthe patient and consistent with Central Health's policies, mission, and goals. Maintains knowledge of community resources and other benefits necessary to refer patients for services and to advocate for patient needs, priorities and goals.
-Supports Central Health's Behavioral Health Program's continuous quality improvement and utilization management initiatives and activities to ensure high-quality standards of patient care.
-Attends and participates in scheduled staff meetings, supervision sessions, training, and other relevant meetings necessary for the clinical, administrative, and operational functions of the Behavioral Health Services program.
-Completes all direct and indirect patient care documentation and services under professional standards, program procedures and relevant local, state and federal regulations.
-Facilitation of mental health education for staff, to support knowledge and understanding of trauma-informed care and behavioral healthcare services provided by CH's Behavioral Health Department.

About the Role
The Social Work Intern Manager is a key member of the Neighbor Care Team at Community First! Village. This role is responsible for leading, mentoring, and supervising a team of social work interns who walk alongside our formerly homeless neighbors. The Social Work Intern Manager ensures interns are equipped to provide direct care, emotional support, and resource connections while creating a meaningful and values-aligned learning experience. This role also serves as a bridge between interns, Neighbor Care staff, university partners, and community resources.

Duties & Responsibilities

Assist interns in providing trauma-informed, person-centered care to assigned neighbors.

Support interns in managing complex needs related to health care, recovery, housing stability/neighbor retention, eviction prevention, and community integration.
