Expected starting wage range is $39.59 - $48.90. Telecare applies geographic differentials to its pay ranges. Compensation:

Definition:
Under supervision, plans and implement medical social service delivery programs in clinical service areas; to perform responsible medical social work for patients at Kern Medical and for their families; and performs related work as required.
Distinguishing Characteristics:
Medical Social Worker I is the first level of the Medical Social Worker series. This class is distinguished from the next higher class of Medical Social Worker II primarily by the amount of direction and assistance they receive in their work. Incumbents who meet the minimum qualification for Medical Service Worker II are eligible to be considered for promotion on a merit basis subject to recommendation of the Department Head and approval of the Director of Human Resources.
Essential Functions:
Dependent upon assignment:
Functions at a highly skilled level in such areas as counseling, medical social work, protective services, family services, community organization and research
Interviews and counsels patients and relatives in order to determine appropriateness of services, to determine extent of social and emotional problems, and to plan for patient's illness and related problems
Evaluates problems and makes treatment plans; Provides casework and group work treatment for patients
Interprets diagnosis and prognoses to patients and their families to insure best adjustment to hospitalization and/or medical condition
Counsels patients on rehabilitation and employment programs
Performs assessments and provides information for use in diagnosis and treatment
Participates in research projects, mental health educational programs, treatment intervention and assist in discharge transition
Observes effects of current policies and informs supervisor of gaps in the program
Participates in conferences, meetings and committees

Employment Standards:

Assess identified members to determine appropriate members for management early in their disease process and at any time during the continuum of care.
Complete a comprehensive assessment to identify patient risk and develop a care plan utilizing expertise and judgement to evaluate needs for alternative services as needed.
Assess members' Social Determinants of Health, such as housing, food, transportation, and safety in the home.

Job Duties and Responsibilities:
• Manage and maintain a caseload of PHM members, with a primary focus on those with behavioral health and social needs, while coordinating with the care team on medical needs as appropriate.
• Conduct comprehensive assessments and develop individualized, person-centered care plans in collaboration with members, caregivers, and providers.
• Support members with behavioral health needs, including serious mental illness (SMI) and/or substance use disorders (SUD) through linkage to appropriate services.
• Provide culturally appropriate education to members and caregivers regarding behavioral health, chronic disease self-management, and community resources.
• Track member outcomes and document all encounter, interventions, and care plan updates in the Case Management (CM) system in compliance with organizational standards.
• Build and maintain collaborative relationships with providers, community agencies, and social service organizations to facilitate referrals and care coordination.
• Engage members using evidence-based approaches such as Motivational Interviewing to promote collaboration, increase member activation, and improve self-management skills.
• Provide brief crisis intervention and warm hand-offs to appropriate resources as needed.

The Role:
The Licensed Clinical Social Worker (LCSW) will be responsible for providing ongoing support to individuals living with dementia and their families, working as an integral part of the Rippl Care Team. This role involves providing direct services to families throughout the journey of dementia and working collaboratively with an interdisciplinary team to ensure high-quality, compassionate care. Although this role does not provide long-term therapy, it encompasses a wide range of services that ensure holistic and compassionate support for dyads on their dementia care journey.
Essential Functions:
As a member of an interdisciplinary team, guide dyads on the journey of dementia by providing support, strategies, education and resources as indicated by need and by program requirements.
Provide a range of virtual services, including psychosocial assessment, dementia and behavioral health screeners, therapeutic interventions, complex care management and crisis support to inform and support the dyad's care plan.
Support caregivers and patients on issues related to dementia care, through psychoeducation, coping skills, self care education and other solution focused brief intervention.
Manage complex cases and provide consultative support for dyads requiring a higher level of support.
Build strong therapeutic alliances with dementia patients and their caregivers, fostering trust and emotional safety.
Respond to crisis and safety concerns and follow all relevant mandated reporting guidelines.
Provide coaching and educational support for the Care Navigation team.

The Role:
The Licensed Clinical Social Worker (LCSW) will be responsible for providing ongoing support to individuals living with dementia and their families, working as an integral part of the Rippl Care Team. This role involves providing direct services to families throughout the journey of dementia and working collaboratively with an interdisciplinary team to ensure high-quality, compassionate care. Although this role does not provide long-term therapy, it encompasses a wide range of services that ensure holistic and compassionate support for dyads on their dementia care journey.
Essential Functions:
As a member of an interdisciplinary team, guide dyads on the journey of dementia by providing support, strategies, education and resources as indicated by need and by program requirements.
Provide a range of virtual services, including psychosocial assessment, dementia and behavioral health screeners, therapeutic interventions, complex care management and crisis support to inform and support the dyad's care plan.
Support caregivers and patients on issues related to dementia care, through psychoeducation, coping skills, self care education and other solution focused brief intervention.
Manage complex cases and provide consultative support for dyads requiring a higher level of support.
Build strong therapeutic alliances with dementia patients and their caregivers, fostering trust and emotional safety.
Respond to crisis and safety concerns and follow all relevant mandated reporting guidelines.
Provide coaching and educational support for the Care Navigation team.
Collaborate closely within an interdisciplinary team, working alongside physicians, ARNPs, LCSWs, and Care Navigators to deliver holistic care.
Drive learning, development, and quality improvement, contributing to ongoing enhancements in dementia care delivery.
Participate in review of clinical issues, documentation and care delivery content.
Assist with care coordination of patient care with primary care, specialists and home health aides
Identify and connect dyads to appropriate resources, services, and support networks in the community.
Return all dyad communications within expected timeframes.
Maintain timely, accurate and detailed records of patient assessments, care plans, interventions, and outcomes, adhering to documentation requirements and relevant privacy and confidentiality regulations.
Support continuous learning of interdisciplinary teams by sharing new resources, services and materials identified during care coordination.
Participate in initial and ongoing training about neurodegenerative diseases, caregiving and other topics as required.

Qualifications:
Must reside in one of our states of operation (WA, OR, CA, TX, MO, IL, FL) and hold an active, unrestricted LCSW license in both your state of residence and in CA, WA, or OR
Unrestricted ability and willingness to license in 49 other states per business needs.
3+ years of experience providing care management and behavioral health services for dementia, palliative care, psychiatry or geriatric settings preferred.
Previous experience providing virtual services strongly preferred.
Ability to work collaboratively with interdisciplinary team
Passionate, empathetic and patient-centered focus that supports and advocates for patients living with dementia and their families.
Cultural humility and the ability to work with diverse populations
Knowledge and competency of evidence-based practices that support work with the geriatric population. Certification in Solution Focused Brief Therapy a plus.
