We provide service at 3 locations in Philadelphia, PA, managing the health and wellness of patients from infants through senior citizens. We practice medicine as a partnership between us and our patients. POSITION PURPOSE: Performs patient mental health assessments and treatment plans and conducts individual, group, and family therapy. Crisis counseling and intervention under the direction of the Psychiatrist/Behavioral Health Coordinator. ESSENTIAL FUNCTIONS AND BASIC DUTIES: Conducts mental health evaluation/assessments for patients referred to behavioral health. Conduct individual, group, and family therapy for all patients as indicated. Conduct crisis counseling and intervention and formulate comprehensive treatment plans. Be able to assess patients requiring 302 services and/or are suicide risk and refer as indicated. Contact and coordinate the Mobile Crisis Team for all patients requiring services and provide follow-up for all patients transported by the Mobile Crisis Team. Document all patient assessments in the medical record in accordance with Community Behavioral Health (CBH) guidelines. Prepare and complete progress notes in Subjective, Objective, Assessment, and Plan (SOAP) format with all required information, including the patient's clinical status, mental status, summary of assessment of the patient's progress and treatment plans, and diagnostic information. Conduct biopsychosocial assessments, formulate accurate diagnoses, and develop and apply appropriate treatment plans, including behavioral management strategies, in accordance with CBH guidelines. Participate in Treatment Team meetings. Comply with all Health Insurance Portability and Accountability Act (HIPAA) policies and procedures. Comply and adhere to all policies and procedures outlined in the CBH Provider Manual. Participate in Utilization Review/ Quality Assurance (QA) activities. Maintains required yearly Continuing Medical Education (CME) credits. Participates in and completes all CBH mandatory in-services and participates in at least ten (10) hours of documented in-service per year. Adhere to QCHC policies and procedures. Adhere to QCHC House Rules. Performs other duties as assigned by the supervisor. Qualifications KNOWLEDGE, SKILLS, AND ABILITIES: Fundamental knowledge of behavioral health and the provision of mental health services. A deep understanding of how illness, disability, and injury impact a person's physical, mental, and emotional well-being. Knowledge of how to navigate different healthcare settings and the broader social service system, including insurance, eligibility for benefits, and community resources The ability to assess a client by evaluating their biological, psychological, and social factors. This includes understanding the impact of substance abuse, mental health conditions, and trauma on a person's health. An understanding of how illness affects family relationships, roles, and communication. Knowledge of ethical principles in social work, patient rights, confidentiality (HIPAA), and standards for end-of-life care. Sensitivity to and knowledge of diverse cultural, ethnic, and religious backgrounds and how they influence a patient's beliefs about health and treatment The ability to coordinate care, manage caseloads, and create, implement, and monitor a patient's treatment and discharge plans. A talent for de-escalating emotional situations, assessing immediate safety, and providing support during a crisis. The capacity for critical thinking to analyze complex situations and develop effective, individualized solutions.
