What You Will Do
Delivers outreach services to mentally ill individuals in the community, providing in-home mental health intervention to those experiencing or at risk of psychosocial difficulties
Collaborates with another team member to perform psychosocial evaluations and assess mental health service needs of clients and their families through observation and interviewing
Creates and execute short-term service plans for clients in collaboration with an interdisciplinary team
Responds promptly to dispatch calls, traveling from the office to the clients' homes within two hours
Ensures seamless program operations as a vital liaison to community agencies and stepping in for the Program Coordinator as needed
Ensures uninterrupted comprehensive care from counseling and care management to long-term supplemental treatment referrals and follow-up
Prepares case histories and prepares and maintains case records, in accordance with the Program record-keeping mechanism
Encourages resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare
May provide clinical supervision for Mental Health Technician, graduate Social Worker students or junior staff Social Workers
Qualifications
Current registration to practice as a Licensed Master Social Worker in New York State Required as determined by operational/regional needs
Valid driver's license may be required

RESPONSIBILITIES
The Social Worker will provide community based mental health treatment to a caseload of 20 children/youth and their families.

The Licensed Clinical Social Worker provides direct clinical care to patients in our Outpatient program in the therapeutic process from admission to discharge, and provides case management, assessments, group therapy, preparing discharge plans as indicated individually for each patient. Individual therapy and family work may also be involved in the position. The Social Worker will interface with internal and external stakeholders to ensure patient-centered, evidence-based, high-quality care for patients.
Duties/Responsibilities:
Carries a clinical caseload, conducts family therapeutic meetings utilizing biological, psychological, and systemic perspectives in understanding patients and their families to develop treatment plans and interventions from those perspectives appropriate to the ages of patients served.
Works cohesively with other team members and is a contributor to the team; actively communicating with the Director of Social Work and other Social Workers, as needed. Ensures care provided is appropriate to the ages of the patients served.
Holds self-accountable to helping the patient and family achieve desired results, understanding and identifying professional strengths, limitations, and challenges.
Actively participates in clinical rounds & social work staff meetings/trainings.
Facilitates discharge planning and implementation.
Conducts group therapy sessions with techniques appropriate to the ages of the patients served.
Provides intern and/or staff supervision, as requested by the Director of Social Work or Director of Clinical Operations.
Performs other duties as assigned.
Required Skills/Abilities:
Must have the ability to function optimally in a stressful environment, and the ability to remain calm in emotionally charged situations.
Experience with Dialectical Behavior Therapy (DBT) and other evidence-based therapeutic modalities.
Proficient in group work and family systems approaches.
Strong experience in comprehensive assessment within a mental health context.
Education and Experience:
Master's degree in Social Work or Mental Health Counseling required.
LCSW in the State of Connecticut required, New York preferred.
Experience working with a psychiatric patient population required.
Physical Requirements:
Ability to move light objects weighing less than 20 pounds for short distances.
Ability to sit at a desk and walk around for different periods of time.
